I can't find a table anywhere... can someone tell me the source and page for it please?

right, but at what levels do you gain wild shape and fast movement? is it 5 just like a druid?

It would be level 5, as a Druid. There's no official table or anything, but

At level 1 you gain Fast Movement
At level 5 you gain Wildshape 1/day
At level 6, 7, 10, 14, and 18 you gain +1 Wildshape/day

You do not gain Large forms at 8th, Tiny forms at 11th, Plants at 12th, or Elemental Wild Shape at 16th.

It's 'as Druid' and 'as barbarian'. So, lacking any contradictory text, it gains them at the same levels as those classes do (so 5th and 1st)

There isn't one, because there's no need for it. You don't need to reprint a whole classes table to change two minor features. No ACF I know of does that (and, despite calling them class variants, they are just ACFs).
