You have begun to ascend beyond the limits of mortality, and arguably even divinity.

Requirements:
Abilities: Manifesting ability 20 or higher before magic or psionic item bonuses
Skills: Knowledge (Psionics) 25 ranks, Psicraft 25 ranks
Class Features: Twofold Master class feature, Manifester Level 21st or higher, ability to manifest 9th level powers
Special: You must have used true mind switch at least once and your original, birth body must be dead.

Class Features:
Note: When a class feature says Believer and does not specify otherwise, a Thrall also qualifies.

Manifesting (Ex): Every level in this prestige class increases your manifesting level in one psionic class of your choice.

Thought Link (Ps): Starting at 1st level, you can, as a standard action, initiate telepathic communication with any Believer from your Thrallherd class feature so long as they are on the same plane. While doing this, you know their surface thoughts. You can only take move actions while maintaining this communication; there is no limit to how long or how often this can be done.

Epic Thrallherd (Ex): Starting at 2nd level, you have Believers and Thralls per the Epic Leadership feat's chart (your second thrall is still unable to be higher than your level -2), however you now calculate your Leadership Score using your character level + your manifesting ability modifier (note your Thrallherd levels no longer count twice). You still ignore all other Leadership Score modifiers.

Sensory Link (Su): Starting at 3rd level, when using your Thought Link, you can choose to see through your Believer's eyes and hear through their ears. If you choose to do this, you are blind and deaf to what is going on where your body is (unless the Believer can see or hear the same area, but it is from their perspective, not yours). If the Believer you whose senses you are using is subjected to an exorcism or other ability or spell that can expel a possessing entity while you are using their senses and you fail the appropriate opposed check you lose access to the Believer and cannot regain access for a number of rounds equal to 1 + the difference of the opposed check, and are also dazed for 1 round.

Status Sense (Su): Starting at 4th level, you always know the condition and location of every Believer you have so long as they are on the same plane, as though they were all subject to a status spell cast by you. Your Believers may also alert you if they feel threatened.

Body Control (Su): Starting at 5th level, when using your Sensory Link, you can choose to take complete control of the Believer's body, guiding the Believer's every action. If you do this, you cannot move your body at all and are completely unaware of what may be happening to or near it, and provided you are not using Multi-Link you can take standard and full-round actions through your Sensory Link Believer (including manifesting powers). While controlling a Believer in this way, their HP, ability scores, saving throws, and racial features tied to the physical body still apply as appropriate, except you can use your skill modifiers (with the exception that you must replace your modifiers from Strength, Dexterity, and Constitution based skills with their modifiers) and psionic abilities (benefiting from your manifesting ability modifier instead of theirs) through them. If you manifest mind switch or true mind switch while controlling a Believer in this way, the victim and the Believer switch bodies. If the Believer you are controlling is subjected to an exorcism or other ability or spell that can expel a possessing entity while you are controlling them and you fail the appropriate opposed check you lose control of the Believer and cannot regain control for a number of rounds equal to 1 + the difference of the opposed check, and are also dazed for 1 round.

Multi-Link (Su): Starting at 6th level, you can use your Thought Link on a number of creatures equal to your manifesting ability modifier at the same time. You can still only use your Sensory Link on a single creature at a time.

Willing Possession (Ex): Starting at 7th level, you can manifest true mind switch on any Believer at any time through your Believer Thought Link without any XP cost; your Believer will always voluntarily fail the Will save.

Multi-Sensory Link (Su): Starting at 8th level, you can now use your Sensory Link in all of your Multi-Link subjects, and you can use any senses your Believers have (including smell, touch, etc). While doing this, you are totally unaware of the surroundings of your body or what is going on to it unless you take damage. You can still only use Body Control in a single subject at a time.

Multi-Planar Link (Su): Starting at 9th level, all of your Hive Mind abilities work on all Believers regardless of which plane of existence they are on, unless special planar geometry prevents it (for an example, the Far Realm is right out).

Fast Telepathy (Su): Starting at 10th level, you can use Thought Link on any number of Believers as a free action which does not take any action to maintain; your Multi-Sensory Link and Body Control abilities still require you dedicate actions to use and maintain them as appropriate.

Overmind (Ex): Starting at 11th level, you are no longer limited to a single body, instead existing as the group consciousness of your Believers. You can only act through your Believers unless you have been exorcised (see below), you no longer have any racial features tied to a physical body, your physical ability scores only exist while you are exorcised and are equal to your Charisma score, you are no longer subject to age, you are considered an Outsider, you do not need to sleep and only need 4 hours of relative inactivity (defined as not manifesting a power or using anything other than Fast Telepathy) to regenerate power points and similar abilities, and you can only be targeted by effects that have no maximum range. You can no longer use your Willing Possession ability. If you are not using Body Control and have not been exorcised, you can only manifest Clairsentience powers that do not have a maximum range, or powers which are targeting your Believers (you are considered to be touching all of them at all times).
If you have class features tied to a physical body (such as the powers of the Crystal Master prestige class), you can only use them while using Body Control on a Believer who has been prepared appropriately. If you have any passive abilities (such as class features or feats) that protect you from Divination or Clairsentience effects, all of your Believers now benefit from the feature in question. Any effect that detects, reads, senses, probes, or otherwise reveals thoughts that is used against one of your Believers will overwhelm the user of the effect and daze them for 1 round while only informing them that there is something alien present, unless the attempt is made while you are manifested due to an exorcism. If all of your Believers are killed, you manifest as if you were exorcised until you have at least two Believers again at which point you discorporate.
If someone attempts to exorcise one of your Thralls or otherwise uses any spell or other ability that could force a possessing entity to leave the Thrall's body, you make any appropriate opposed checks applying your manifesting ability score modifier as a bonus to the check. If you lose, you manifest in the Thrall's space as an incorporeal being with any appearance you choose of the same size as the Thrall you were just exorcised from for a number of minutes equal to 1 + the difference in the opposed check, and you are dazed for one round. While manifested in this way, you cannot manifest the mind switch or true mind switch powers, you apply your Charisma bonus (if you have one) as a deflection bonus to your AC, and have a fly speed of 30 feet with perfect maneuverability. If an exorcism or similar effect is made against a Believer who is not a Thrall and who is not subject to your Body Control or Sensory Link, you make any appropriate opposed checks. If you fail, you are dazed for 1 round.

Note: Inspired by Fiend of Possession shenanigans suggestion of possessing a body of water and having a bunch of things drink it when I asked about how to do this without homebrew. Also the spell soul link and the Overmind of StarCraft.

I don't normally play epic-level, so take any of my comments in that context:

PrC is flavorful and fun.

You should probably specify which abilities are (ex), which are (su) and which are psi-like.

Multi-Planar Link seems potentially setting breaking (e.g. "no one comes back from a specific demi-plane, I'll just send a believer there to find out why"). On the one hand, epic level things breaking settings may not be so bad. But I think it might be reasonable to narrow this ability so it only applies when believers are on planes which are normally accessible such as via plane shift or portals.

So, if I read this correctly, the capstone prevents you from using any of your offensive powers, ever, unless you're exorcised? Which is not very often? Certainly, it is powerful to have direct control over so many bodies, but if your only new options with those bodies is buffs (which a Telepath is not usually focused on), then that's only slightly better than having the followers in the first place.

Certainly, it is powerful to have direct control over so many bodies, but if your only new options with those bodies is buffs (which a Telepath is not usually focused on), then that's only slightly better than having the followers in the first place.

Negative. It means you cannot use such powers unless either you are exorcised or you are using Body Control on a Believer/Thrall. Note that BC is still a single-subject ability; your near-omnipresence only applies to accessing the thoughts, and to a lesser degree the senses, of your Believers. So, you pick a Believer or a Thrall to use as an avatar, and if it dies, you switch to another one. Thrallherd will automatically replenish lost Believers and Thralls, albeit there will be a short wait so you don't want to run down your membership too quickly.
EDIT: Also, added what happens if all of your Believers are killed.

Okay, I see now. What exactly constitutes a Believer that has been 'prepared appropriately' to use physical class features?

Crystal Master features (already listed as an example) require appropriate crystals be embedded in your body; to use them through a Believer, said Believer must have such crystals embedded. I would also say Impure Prince PrC features require they have the appropriate symbiont, etc.
