What does the Soldier Cavalry specialty do you for you? Does it give you proficiency in horseback riding or is it just fluff?

The specialties don't give you anything, per se; they are there to explain skill, attribute, and characteristic choices. Choosing the cavalry option would give you a justification to putting points into Animal handling as a racial or class skill. It's just about giving your character flavor and making them unique.

The specialty does nothing on its own, however you are allowed to customize your background. If you want the specialty to mean something, you might exchange the Athletics skill for Animal Handling (you have learned how to work with animals through working with your steed, but you haven't developed the endurance and athleticism of the soldiers that have been marching and climbing fortifications.

Similarly, a healer or scout might trade Intimidation for Medicine or Perception, a quartermaster or support staff might trade the gaming set for an appropriate tool, and an posh officer might trade Athletics for Performance as he lived a life of luxury while raising the morale of the troops.
