You confer on the subject the ability to see all things as they actually are. The subject sees through normal and magical darkness, notices secret doors hidden by magic, sees the exact locations of creatures or objects under blur or displacement effects, sees invisible creatures or objects normally, sees through illusions, and sees the true form of polymorphed, changed, or transmuted things.

So you see things as they actually are. Do you also see things the way they are portrayed or at least know that they are being portrayed as such?

For instance, Bob has a Hat of Disguise on making him look like Jill. With true seeing you obviously see it is Bob but do you also see that Bob is trying to look like Jill? if someone is invisible you see them but do you also know they are invisible? And etc.

Yes. It says you can see things as they actually are. However, it never says anywhere that you stop seeing things as they aren't, so you still do that as well.
