Lore Question

Okay, so I'm having some serious trouble remembering this bit of lore and I can't seem to Googlefu my way to an answer. Hoping one of you guys remembers and can help.

Basically at some point Bahamut made an avatar that was much like Tiamat, only with three heads instead of five. I'm trying to remember something, anything about it. A name, the event, whatever it was that caused this to happen, if anybody has a link to some information and maybe even a picture it would really help.

Nah, there's no 3-headed Bahamut. There are a couple of 3-headed Tiamats. The entry for purple, yellow, and orange dragons makes a reference to a lost sister of Tiamat with those 3 heads. There's also that Tiamat was often depicted as a 3-headed dragon in Sumerian mythology before Gary Gygax changed her up to a 5-headed form for his game. Back then though, I think her main enemy was Marduk, god of the City. There was a whole Ghostbusters cartoon episode about it.
