Default answer for living creature is wand of lesser restoration which costs little and require no special investment in character. Which is more efficient then spending reserve feat for trivial task. What is default for undead?

I always could use wand of Inflict Light Wounds, so it is the cap of efficiency. Is there any other means for undead healing? D&D designers meticulously erased all possibilities for infinite healing, so wands become the cheapest solutions (not to forget belt of healing). What about inflicting negative energy? It likely avoided attention focus.

Black Sand, from SandStorm p20. Keep a pouch on hand and use it whenever you have downtime for undead fast healing. Also good for killing foliage, healing yourself if undead, emergency weapon against swarms, and keeping pests away from your gear.

One level of Dread Necromancer? Charnel Touch is only 1hp per touch (+1 per 4 class levels), but it's still infinite out of combat healing for undead (and people who have the Tomb-Tainted Soul feat).

The Rod of Enervating Strike (from Magic Item Compendium) offers a sideways method of healing undead. It functions as a magical heavy mace, but deals an Inflict effect if it connects. Take the -4 penalty to make your strikes nonlethal; the undead will be immune to the nonlethal damage but the inflict effect still connects. Your zombies will be healthy in no time, and you'll get to relieve some stress by beating on them with a stick. The only catch is the price tag; a bit over 18,000 to purchase.

Not sure how "cost-efficient" it is for your tastes, but a deathlock (Libris Mortis, pg. 94) is a 3 HD undead that has Inflict Minor Wounds as an at-will SLA.

You can track one of those down, or if you are any sort of necromancer ask your DM if you can create one yourself maybe?

One of those in your control pool is basically unlimited healing.

You can get infinite out of combat healing for your undead minions and your teammates (!) with a single lvl dip into Shadow Sun Ninja.

The "Touch of the Shadow Sun" ability lets you do negative energy dmg (or heal on undead targets) on the first use and does positive energy heals (or dmg on undead targets) on the second use.

Out of combat you can alter between positive and negative energy heals al long as you have the right targets to heal.

To increase the heals, you can use a Necklace of Natural Weapons with Sizing (due to heals scaling with unarmed dmg; size your hands to colossal size for extra heal) and a Monk's Belt.

Huh? Erased from where? Also, that is simply not true. Reserve Feat, Dragon Shaman, Crusader to Name a few rather straight foreward sources of unlimited healing.

Don't forget templates that give Fast Healing.

Any class that can channel or inflict neg energy with a touch will do, but that is limited.
In pathfinder at least, a lich can inflict negative energy with a touch and heal themseleves or other undead creatures at will.

Necrotic Touch (SU) If a Necrosis Carnex hits a living Creature with its touch attack, it deals damage equal to 1d6 + 1 per 2 HD of the Necrosis Carnex. Undead are instead healed by the same amount, gaining any hit points over their full normal total as temporary hit points that last for up to 10 minutes. ...

A spellcaster of 11th level or higher can create a Necrosis Carnex with an animate dead spell. To do so requires three corpses from Medium creatures and cold-hammered iron bands worth 200 gp. None of this material is consumed in the casting and but instead becomes the undead amalgam of the carnex. When used to create a Necrosis Carnex, the animate dead spell has a casting time of 10 minutes.

Fun little creature with a few decent abilities, just dont let the paladin see it. it has 4 starting HD so its 1d6+2 and if its killed it goes boom for 1d6+2 negative damage to everything within 30 ft. Your still creating it so all your normal undead stuff work on it. I'd say 200 GP for unlimited use is fairly cheap.

Also eternal wands will assist

I also thought about necrotic carnex. It is cheap to animate, but you need some option to store it. So I added enveloping pit to the cost of this healing method.
