Elemental Wild Shape

Two questions

1) Do you need to have seen the elementals to be able to wild shape into them?
2) is the elemental wild shape at 10 based on CR, or you can just turn into them?

It allows the druid to turn into one of the 4 elementals. There are no further restrictions beside requiring 2 wildshape charges.
