In another thread the idea of going from a D&D-verse to Poke-verse came up.

Using as much canon material as possible how best would one traverse their favorite novel/tv/game universes using a D&D-verse as a starting point?

Would you mind explaining what you are actually asking? Because if you're asking "How do you play Pokemon using D&D rules" is completely different from "How do D&D characters walk in to the Pokemon world." ....as the answer is clearly rule 0, since the two are from completely different canons and do not coexist.

Literally asking about travel. Pretend that a real world pokefanatic got transported to D&D-verse. Now that they have access to magic how do they skip realities and go pokemon-ing instead.

Some ideas...
The Plane of Dreams could easily have some analagous pokemon-like dream creatures.
IIRC the Dreamheart is supposed to be a pretty potent conflict of reality warping energies.

The Shadow Plane is supposed to connect everywhere IIRC. Wander deep enough into Deep Shadow and it's hard telling where you'll wind up.

Sigil is an easy one. One moment you're browsing wares in an archway in the City of Doors, the next you're popping out of Hoopah's ring.

That sort of thing.

Any of the existing planar travel mechanisms would work fine.

Essentially, yes. If you have a cosmology wherein they are connected in some way.

Step 1: notice there is no pokemon universe in your cosmology(and that your gm is screaming no at you with all the air it can get and also beware of flying books)
Step 2: become a level 21 wizard or cleric(preferably of no god) or druid or stp erudite or whatever T1 class you want.(more flying books coming at you when you pick epic spellcasting)
Step 3: use epic research to make an origin of specie spell for each kind of pokemon and use some sheanighans to make a very quickly growing demiplane(for example by using the fact some versions of genesis does not says the additional growth from extra casting is done at any pace and only says "grows X more foot" (which suggests it is instantaneous furthermore the spell duration is instantaneous) which means that you can just get 50000000000000000 casters to cast genesis to grow your plane at once and immediately have an absolutely gigantic demiplane) (meanwhile if you do not have a very strong support from the other players the gm probably kicked you out of the group and considers your reading of raw shaky)
Step 4: create a planet and populate it with pokemons and humans and make some epic spell which turns kids insane and make parents go along the insanities of their kids(and which also make pokemon willing to fight and makes pokeballs possible to create). (creating planets is quite trivial but if you want some lessons about terraforming you can go around in gitp)
step 5: discover suddenly that it now makes way more sense than the original pokemon universe while being surprisingly close.
Step 6: now you can go to pokemon street since you created it.

There is no such thing as impossible in dnd 3.5 (addendum: provided you have immunity to flying books and support from all the other players)

Let's see... can we do it with less than Epic spellcasting...

Pokeballs could be refluffed Iron Flasks (http://www.d20srd.org/srd/magicItems/wondrousItems.htm#ironFlask)

Refluffing regular monsters works just fine for Pokemon (there's a lot of them).

After that, it's just a matter of making everyone act appropriately to the genere. Mass Suggestion, Dominate Monster, Geas/Quest, Mark of Justice, and so on might do the job.

Except that here we were talking of what a player could do.
Gms are allowed to refluff his own monsters.
Players are not allowed to.
Therefore it simply does not works at this step.
Iron flasks costs way too much and are magic items so the people in that universe will not be able to produce those themselves without using magic and it would make the universe stop being the pokemon universe(and if you produce yourself the iron flasks and distribute them then it will again not be like the pokemon universe because people would be in a world where they get pokeballs nobody at all understands but some spellcasters).
You will have an universe similar to the one of pokemon but it will not be the same and people could as well say it is like digimon or yet one other pokemon like universe but it will surely not be the pokemon universe for the creatures will not be the same ones.

However if you are ready to use regular spellcasting and a lot of ice assasins you can make something that looks like the pokemon universe(but which is not) by spamming illusions but then there would not be real pokemon and only ice assassins of wizards using illusion spells to pretend to be pokemons and stuff like that.

Step 3: use epic research to make an origin of specie spell for each kind of pokemon Mew and/or Arceus...

Had to amend that bit for ya. Pokemon canon is all things evolved from and/or were created by Arceus. They are effectively the primordial ooze and god, respectively.

Ok makes sense.
So now you have to create and mind rape them to make them create all the pokemon.
