Im building an adventure with DnD like mechanics and i want help with plot and twists.
Setting is modern real world with a high level of masquerade.
I want to hear things like vatican is run by vampires in disguise. Banks ceo are dragons, fey populate the amazonas.
As much detail as you would like. But i need help with this. Thank you all.

Druids and fey working as environmentalists. Good fey (pixies, nymphs, etc) might try working with the government or NGOs, neutral/evil fey might be ecoterrorists (or conversely might "sell out" and work for polluting corporations).

Dwarves, gnomes, and perhaps even goblins working as engineers or in other technical industries.

Lillends, bards, djinni working in media, Hollywood, artsy cafes, etc.

Outsiders (both good and evil) influencing politics.

I like the idea of Trump being an evil outsider looking for influence to search for an artifact on asia

Even better. Have "evil" corporations employ druids (environmental scientists/engineers) to do good environmentalist stuff so that those corporations get tax breaks and don't get sued...

Just like the real world :)

-Governments of the world (or all major political parties) are being run in the background by Mind Flayers and all "elected" officials are actually thralls.
-Demons or Devils are behind the world's large religions
-Merfolk are actually causing the ice caps to melt so that more of the world is covered in water and they can take over
-The Red Cross is actually run by Vampires
-Television actually can cause people to turn into zombies (our Mom's were right!!)
-The Horror Movie franchise system is run by liches to desensitize us to undead and not be afraid of them
-The Mohedrons are behind the industrial revolutions and computer age.
-Some famous sci-fi book or TV show is actually a history of what truly happened instead of fiction
-The college system is trying to persuade intelligent people to study mundane things instead of becoming wizards that can challenge them (maybe Red Wizards of Thay)

Look to the old Disney show "American Dragon: Jake Long" for inspiration. This is basically the premise behind the show. Urban Fantasy with classical fantasy tropes mixed in.

The otyugh/ooze lobby backs oil and single-use plastics, and supports a thriving civilization on the Great Pacific Garbage Patch. When asked to comment on recyclables, one member commented "It's just not the same."

All of this is good stuff, keep it coming!

But don't be afraid to be more specific, and less vague. Think it as a sidequest.

A local politician missioned the heroes to inquiry onto her son's new girlfriend, because it's all sudden and he's deeply in love with her. She saw once that the girl has what she thinks to be an amulet of antidetection and she believes it's some kind of demonic succubus or an enchantress that uses her son to get to her.

The thing is, the girlfriend is not at all fill with bad intentions here. It's a young hag that fled from her coven and lives now in disguise to avoid being caught back. The love between them is genuine, despite being based on a false appearance and lies.
The hag might become aggressive to protect her secret, but it's not the worst: if her amulet is off for a moment for some reason (taken off, dispelled...), the coven manages to track her down and try to get her (killing people that stay in their way).

About two upper managements ago, I figured the most logical reason for their actions was that they were unseelie fae feeding off negative emotions and crushed morale.

Have a business where management seems to be actively crushing morale. Examples include absurdly enforced dress code (e.g., no open-toed shoes in an office environment -- really annoyed a lot of ladies in the office), memo saying we should not decorate our offices or feel comfortable, statements that working late is appreciated but will not be compensated for (since you choose to stay, coulda gone home, and it's still your fault if you miss the deadline), not being allowed to use one of the elevators because about 5% of the time someone needs it for a special purpose.)

Plus layoffs and promises of more layoffs.

*Internet trolls are actual trolls
*The major political parties are part of a feud between two archfiends
*The third world is under the thumb of the four horsemen of the apocalypse
*Vignette spoofing Star Wars in which Bigfoot is a drug smuggler
