Any non-homebrew setting. I am curious.

Say you've got a Worshipper of [evil god] dressed up as one of [good god], and slowly corrupts the masses to [evil god], who they think to be [good god]. Or vice versa. How likely is this guy to be smited in to the nether realms?

What about a someone who spreads [infectious disease] in a town, then dresses up as a practitioner of [god of healing] to collect money from the sick and dying to cure their diseases?

Eberron - no reaction from the gods, if they even exist; the church is expected to police itself.

Forgotten Realms - the god takes offense but other gods stand against the offended one acting so the status quo is maintained forever until Ao decides to wreck everything and release another edition, per the orders of Ao's divine boss.

Greyhawk - the god sends a vision to someone to go take care of that false practitioner, preferably with a sword; there are gods of disease though so the disease-cleric might have an evil guardian angel of some kind.

Ravenloft - everyone expected that it was an evil cleric in disguise the whole time.

Midnight - the evil cleric is the only type of cleric allowed; he doesn't dress up as a good cleric. The infectious disease is regarded as a blessing since it's better to die than keep working as slaves for the overlord.

Dark Sun - there are no gods as such; Clerics petition the elements & para-elements for power, and the elements are neither good nor evil.
