Blind A Maw Dragon, while appearing to have eyes, is in fact totally blind. As such, it's immune to anything that requires it see something, such as a medusa's gaze.

Legendary Resistance (3/Day) When a Maw Dragon would fail a save, it may choose to succeed instead.

Gods, Why? When a Maw Dragon reveals its true nature or it is otherwise made apparent, all who view it within 120' must immediately make a DC17 Wisdom save or become Frightened of it until they make a save. They may repeat the save at the end of each of their turns, but this is made with disadvantage if the Maw Dragon is visible.

Innately Magical All the Maw Dragon's attacks count as magical.

Actions
Flailing Limb Melee Weapon Attack: +15 to-hit, reach 15', one target. Hit: 1d12+9 (15) bludgeoning damage, and the target is grappled.

The Maw Melee Weapon Attack: +24 to-hit, reach 5', one grappled target. Hit: 6d12+19 (58) piercing damage, and the target must make an opposed grapple check against the Maw Dragon. If the Maw Dragon succeeds, it swallows the hapless victim, banishing them into its stomach, an anarchic realm of gnashing teeth and nightmares. A swallowed target takes 2d12 (13) piercing, 2d12 (13) poison, 2d12 (13) necrotic, and 2d12 (13) psychic damage at the start of each of the Maw Dragon's turns. They are also restrained and blinded, and lack line of sight or effect to anything outside the Maw Dragon's stomach. At the end of each turn, the Maw Dragon must make a Constitution save with a DC equal to the damage it took from swallowed creatures that round. On a failure, it regurgitates anyone swallowed within the past minute.

Multiattack The Maw Dragon may make one Flailing Limb attack per limb (usually six-four legs, a head, and a tail) and a single Maw attack.

Bonus Action
Shriek (Recharge 5-6) The Maw Dragon lets out a massive shriek, piercing into the minds of those near it. Everyone within 60' must make a DC 22 Constitution saving throw, taking 2d12 (13) thunder damage on a failure and becoming deafened for one minute, or half as much and no deafening on a success. They must also make a DC 17 Charisma saving throw, or become affected by a Confusion spell.

Legendary Actions
The Maw Dragon gets three a round.

Flail The Maw Dragon make a Flailing Limb attack.
Scream (2 Actions) The Maw Dragon makes a Shriek attack.
Bite (3 Actions) The Maw Dragon uses The Maw.

A Maw Dragon appears, on initial inspection, to be a dragon. A slow-flying dragon, but a big ol' element breathing dragon. Most people never learn more.

The unfortunate few who do learn it is otherwise, though, will see that a Maw Dragon is merely a massive mouth, large enough to swallow a horse whole, inhabiting the body of a dragon. A close look into its eyes reveal nothing, the dragon merely a puppet being inhabited by the insidious hunger.

A Maw Dragon is eternally hungry. They do not make lairs, nor collect treasure. Instead, they devour, eating and eating and eating anything that moves until nothing remains or it is finally slain. An ill-protected kingdom can find itself entirely consumed by a Maw Dragon, if no one recognizes the threat in time. There is no bargaining with a Maw Dragon-while claiming to know where more food lies might get it to stay its mouth for a few moments, it only lasts until the hapless victim talks or tries to wiggle free, after which, they are swiftly eaten.

The stomach of a Maw Dragon is another plane of existence. Each Maw Dragon seems to tap into the same one-possibly the plane of Dreams, but exclusively for nightmares. As such, though, a Maw Dragon can hold entire other dragons in its stomach, where they are slowly but surely devoured.
