Abeils and Formians are the big two as already stated.

Devils kind of? They have telepathy that can impart a kind of hivemind aspect to the way they act.

Shocker Lizards and Vrock Demons also kind of work that way because they have combo attacks that are stronger with more of the monsters present. You'd have to do some modifications to really get the sentient insect feel, but they could work.

Grell may be considered eusocial. Elderbrains give Illithids a structure similar to eusocial creatures they have a lot more individuality than eusocial creatures usually do, but they're individuality is subsumed more and more as they get closer to the elderbrain.
