Its a joke when you read the older editions more. At least, females then to be more Divine Oriented in their spell list, even if they are Arcane Casters. Thus, the Mystic part refers to the Female 4th Level Magic User, while the Theurge part refers to the Thurgist the Male 4th Level Magic User.

The other layer of joke is that a character cannot be a Male Character and a Female Character at the same time, but somehow the class Mystic Theurge recalls being a Magic User of both Gender at the same time.

Actually, Girdle of Gender has 5% Genderless, 5% Hermaphrodite, and 90% opposite sex for male or female characters. It is not clear what happens for the Genderless or Hermaphrodite when they wear the Girdle.
