Civilizations the video game is great at that. Just use an old version with a cheat code to see the whole map right at the outset.

This (http://direpress.bin.sh/tools/world.cgi) is the only one I've ever seen.
It's truly random, but the images are a bit small, and you'll have to add all the details yourself... but it could help.

UPDATE: I've updated that link with a version of the same tool that's been expanded a little.

i use campaign cartographer. you can find it here: http://www.profantasy.com/

it takes a lot of work but it is great, one you get all the software you can build a world map with cities on it then build a city map and link it to the city pic on the world map and even do the inside of a building and link it to the building on a city map.

i am developing a whole earth sized world based on this and filling it in a little at a time.

in my last game on the world map i could click on the city they were in and pull up the city map then click on their tavern and pull up the map of the 1st floor then click on the stairs to pull up the map of the second floor and then the third.

it is a great referance tool with good information and makes great visuals but it costs a little bit and takes hours to put into your maps as they are all made manually.

Pen and Paper with graph paper baby.

I draw it in regions and then tape like 6 sheets together for an super "world map".

Before that, I used multiple images from the "fractal worldmap generator" (Google it). Then put them together in paintshop. Truly random.

There's HexMapper (http://home.paonline.com/zaikoski/ak/TOOLS.HTM)a windows freeware program that makes basic overland hex maps. I use it with Gimp to make maps. Here's a map I recently did of Western Brun (http://pandius.com/mapwbrun.html). There's also a Yahoo group Hexmapper Haven (http://games.groups.yahoo.com/group/Hexmapper_Haven/).

I used a 1 pixel black line to draw the contour of the plateau. Then I drew lines one pixel wide and mostly two pixels long spaced one pixel apart, some of the lines are drawn diagonally.

Looks like another group has decided to work on AutoRealm (http://gryc.ws/autorealm.htm) (it was a dead SourceForge project). Not too bad to work with, plenty of output options.

Examples on the linked page aren't very good, try the sourceforge page for better examples of what can be done with it:
