I'm looking to find a playable race that is in some way associated with disease, or at least that it could be plausible that they have access to disease related magic. I know of the giant pdf list of 3.5 races, but it doesn't really mention anything about the races as far as looks, flavor, etc. as well as only 2-3 races having actual immunity to disease.
This would be for use in a LARP where D&D races can easily be converted, and I'll be playing a character using the path of Pestilence

How about nezumi (ratfolk) from Oriental Adventures?

Ghouls are monsters, but they're low-level and diseased, and could be easily allowed as a PC race with little work. They've also got a savage progression class if that interests you.

Mongrelfolk have strong constitutions, are outcasts, and in fluff are described as living in the gutters and the like, so have a fair chance of going for disease.
If you want to go for a more cartoonish style of cackling eeeeeevil, Vashar and Jerren from BoVD are the vile versions of humans and halflings respectively.

For ghouls you could also use the gravetouched ghoul template for only 2 LA.

There are the Disir from Krynn, the Dragonlance setting; they are statted out in the Revised Bestiary of Krynn. They are worshipers and the favored mortal race of Morgion, the Dragonlance God of Disease and Pestilence-an insectoid race that has been deeply mutated and debased into something pleasing to a God of Disease. The base race is a 5 RHD Aberration with an LA of +2. No Disease Immunity/Resistance, so they can enjoy the blessings of their God; but they have +4 Con and constantly exude toxic slime from their cracked exoskeletons, giving them Fire Resistance and making all their natural attacks poisonous. There are also Queens, who move up to LA +4 in return for better stats and immunities to Poison, Paralysis and Sleep; as well as the ability to mutate their larvae by adding abilities or templates. Mechanically not that great, but dead on fluff wise.

Speaking of mechanically sound: all Draconians from Dragonlance are immune to Disease, and a few have scarce enough RHD to be viable, and the RHD are Dragon anyway. Baaz and Frost are 2 RHD/LA +1 with net -2 and +0 abilities respectively; Kapaks and Venom are 2 RHD/+2 LA with net abilities +0 and +2 respectively, with a natural bite Poison, probably the best bet. The other types have a lot more RHD and LA, albeit with a lot of class abilities like Spellcasting, or Barbarian/Paladin abilities, depending on what you want. Fluff-wise, Draconians were created by the primary Evil Goddess of the setting, to whom the Plague God Morgion is subordinate and allied; so you could jump off there. The most recent updates of the Draconians are in Dragons of Krynn.

Not interested in evil as it's a LARP, but undead I may consider

It may be a bit late, but - since you're OK with ECL-impacting templates - how about the Ghuls from the Dragon #313?
They're the result of their mothers contacting Ghoul Fever during the pregnancy

Also, how about the Feral?
No, not the LA +1 template from the Savage Species - the one from the Feral Animal Companion feat in the Champions of Ruin
It gives +2 on Str and Con, and bite attack inflict Red Ache disease (DC 15 Fort. save)
