Hi there.

There's a supplement for d20 system called Encyclopedia Arcane - Chronomancy. Free content, downloadable.
It has a specific rule for ageing and death for old age ( pg 56 ):


Once a creature has reached Venerable age, it must make a Fortitude save at an interval as shown in the above table. For example, a human must make the fortitude save at 70 years, 75 years, 80 years etc. The DC for the saving throw is 10 but the DC increases by +2 every time the save is made. If a character has any immunity to poisons or diseases, the creature receives a +5 bonus to this saving throw. If the creature has an immaculate body (one that does age but does not suffer penalties to ageing), for example a druid or enlightened, the creature receives a +10 bonus to the saving throw. If the creature fails the saving throw, it dies of old age during the next save interval period (Games Master’s discretion)


So...

could you please post all the ways you know to increase Fortitude throws, possibly stackeable?

I presume that the character doesn't know precisely when these saves will be rolled, and so short-term buffs won't be useful?

This isn't exactly what you're asking for, but if you can anticipate when you have to make the roll, you could cast / umd the surge of fortune spell (from complete champion), which gives you a natural 20 on a saving throw. Therefore you would theoretically live forever. The spell also gives you a +2 luck bonus on saves which is a relatively uncommon bonus type.

Unfortunately, you can't persist this spell, but you could use a spell like divination to determine when to cast it, or if you want to be a little sillier, you could put it in a spell clock.

Steal life from BoVD and Ensul's Soultheft from City of Splendors don't give bonuses to fortitude saves, but they do extend your lifespan.

Superior resistance from spell compendium comes to mind as a long lasting spell that grants a bonus to all saves, as does persistent conviction.

You could either become immortal:

...or you could get a Headband of Conscious Effort from Complete Adventurer. That lets you swap a fort save for a concentration check once per day. You could keep it on full time, pump your skill, and only use it for these aging checks. As a bonus, there's no failure for skill checks when rolling a 1.

Steadfast determination is one way to not fail natural 1s on Fort rolls, which seems important when you're going to be so reliant on them to not die.

Alternatively, you can just stack on rerolls: Alter Fortune spell, Luck Blade, Mantle of Second Chances etc.

The "Mind over body" maneuver combined with taking 10 (which you can always do as long as you're not in combat) should allow you to live a long and happy life by itself.
