Probably 3 or 4 HD, here is a the answer from a PDF that got most of 3.5 / dragon / dungeon but maybe not all of it.

Would a Lycanthrope work? They're Humanoid (or Giant), but their HD are determined by the base animal.

EDIT: I think Legendary Tiger might have the most animal HD, at 26 (Epic enemy).

Well, Tren (Serpent Kingdoms) and Yeti (Dungeon #84) are 4 HD too.

How far into obscure books you want to look?

WotC adaptation of Diablo II - Diablo II: To Hell and Back - have Hierophant (http://classic.battle.net/diablo2exp/monsters/act3-zakarumpriest.shtml) and Zealot (http://classic.battle.net/diablo2exp/monsters/act3-zakarumzealot.shtml) both at 7 HD, and Hell Cat (http://classic.battle.net/diablo2exp/monsters/act2-sabrecat.shtml) - 10 HD.

Bestiary Of Krynn, Revised have Goblin, Cave Lord as 8 HD Large Humanoid.

Dangerous Denizens: The Monsters of Tellene - Deep Stalker Queen is 10 HD Large Humanoid (Aquatic).

I don't think the Dabus has been mentioned yet? 4 HD medium humanoid, Expedition to the Demonweb Pits.

The Multiheaded template can add 2 extra HD to a medium creature without changing its type, or up to 6HD to a large creature. Hornhead Saurials are large humanoids with 2RHD, so a Multiheaded Hornhead could have up to 8 humanoid RHD. If you add lyncathropy into the mix, were-Battletitan adds another 36 RHD (but not technically humanoid HD).

I believe the Mutant Gargoyle in the D&D 3.5 conversion of Tomb of Horrors is listed as having the Humanoid Type and something like 20 Racial Hit Dice. I can't remember the creature's exact name and I long ago lost the pdf that I downloaded when it was freely available from WOTC, unfortunately. Maybe it was called "Four-Armed Mutant Gargoyle?"

I'm fairly sure that it did not have a Level Adjustment or Effective Class Level listed, both because of its pseudo-unique nature and due to its high number of RHD.

Well done...I'd forgotten all about that.

I'm like 99% sure it was supposed to be a Monstrous Humanoid , but the module definitely says Humanoid (CE Large Humanoid [Earth]), and has 21HD.

For templated creatures, probably Incarnate Construct Effigy Half-Dragon LeShay. For base creatures, another one people didn't mention is the Constrictor Naga, which is a Humanoid unless you use the Dragon update.

I cant believe I'd forgotten the Mutant 4 Armed Gargoyle...I had been meaning to exploit the fact it is a humanoid in a Villanous Competition build.
