I think this was in a Dungeon Magazine or early Dragon, and may be 2nd ed. rather than 3rd ed.
Basically the item was a black toadstool with a human face that confers bonuses to the wielder, while attempting to corrupt them into a fungus monster.
Can anyone direct me to the source I might have read this in originally?
