Know what bugs me? That you don't get the tumble bonus to AC when using combat expertise. You should totally get a +1 dodge bonus.
Consider it a house rule.

You realize that combat expertise and fighting defensively are not mutually exclusive, right?

The problem is that you can't use Combat Expertise and a total defense action, because you need to have an attack to apply the Combat Expertise penalty to, even if you choose not attack anyone. You can use Combat Expertise and a partial defense action. And I agree, you should at least gain a +1 to your Combat Expertise.

To fight defensively one must make an attack as well. So one can fight defensively, and go with full combat expertise for a rather big boost to AC that still leaves you with a remote chance of hitting something.

And maybe some deadly defence from CSc, so if you DO hit you get +1D6 damage.
