Regeneration
A Xixecal regenerates 50 HP at the start of its turn. Subtract the amount of HP regenerated by the total of the Fire and Radiant damage it has taken since its last turn, to a minimum of 0.

Abomination
A Xixecal is immune to any effects that would alter its form. In addition, all their attacks count as magical.

Dire Winter
A Xixecal is surrounded by a constant wintery wonderland (of death). Any creature within 1,000' of the Xixecal is subjected to 2d6 Cold damage at the start of their turn. If they have Cold Resistance, the damage is completely negated. In addition, heavy snowfall blankets the area. If a Xixecal stays in one place for at least one minute, the snowfall creates difficult terrain within the 1,000' radius. Finally, sight is obstructed within the radius-no creature other than the Xixecal can see more than 60' in front of them.

Bone-Numbing Chill
Anyone damaged by a Xixecal must make a DC 18 Constitution save, or be affected by a non-magical Slow spell effect until the end of the Xixecal's next turn. Creatures with Cold Resistance have advantage, and creatures immune to Cold are immune to this.

Magic Resistance
A Xixecal has advantage on saves against magic.

Legendary Resistance (3/Day)
If a Xixecal would fail a save, it can instead pass.

Actions
Claw Melee Weapon Attack:
+19 to-hit, reach 50', one target. Hit: 4d12+10 slashing damage, and 4d12 cold damage.

Bite Melee Weapon Attack:
+19 to-hit, reach 30', one target within 30' of its head. Hit: 5d12+10 piercing damage, and 5d12 cold damage. In addition, the creature must make a DC 27 Dexterity save or be swallowed. When swallowed, they are restrained, blinded, and take 4d12 bludgeoning and 4d12 cold damage at the start of each of their turns. At the start of the Xixecal's turn, it must make a Constitution save equal to half the total damage it took between the start of its last turn and now. On a failure, it regurgitates all swallowed creatures. A Xixecal is so big, there's no practical limit to what it can swallow.

Stamp
All creatures within 60' of the Xixecal must make a DC 18 Strength saving throw, falling prone and taking 1d12 bludgeoning damage on a failed save. If they are within 30', the Xixecal can also step on them, dealing 10d12 bludgeoning damage on a failed DC 18 Dexterity save, or half on a success. In addition, on a failed save, the creature is restrained under the Xixecal's massive foot. A Xixecal can restrain any number of creatures, but cannot use Stamp again without letting the creatures go.

Breath Weapon
A Xixecal shoots a 300' cone of cold, dealing 10d12 cold damage on a failed DC 27 Constitution save, or half on a success.

Multiattack
A Xixecal makes up to two Claw attacks, up to one Bite, up to two Stamps, and up to one Breath Attack.

Bonus Actions
Cleave
If a Xixecal slays an opponent with a melee attack, they may make another one.

Reactions
Spell Stowaway
If a creature within the Xixecal's awareness casts a spell that would render them Invisible, the Xixecal may use its reaction to also gain the effects of the spell.

Legendary Actions
A Xixecal can take 5 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature’s turn. A Xixecal regains spent legendary actions at the start of their turn.

Attack. Make one Claw or Bite attack.
Stamp or Breathe (Costs 2 Actions). Make a Stamp or Breath Attack.

Outsider isn't a creature type in 5ed. You probably want Elemental (Titan).

What is the lore behind this beastie?

Whatever the 3.5 fluff was. I just converted it.

Also, fudge, I forgot to slap an AC on him.
