A few years ago, I made a conversion for The Nameless One (from Planescape Torment) for 3.5 (https://forums.giantitp.com/showthread.php?234535-The-Nameless-One-(3-5)). I've finally gotten around to updating it for 5e.

Challenge 30+ (155,000+ XP)Former Player Character. The Nameless One is a former player character and therefore calculates his hit points as would a player character (maximum hit points at his first HD, and taking the average rounded up for each HD thereafter).

Legendary Resistance (3/day). If The Nameless One fails a saving throw, he may choose to succeed instead.

Magic Resistance. The Nameless One has advantage on saving throws against spells and other magical effects.

Regeneration. The Nameless One regenerates 15 hit points per turn. He can also reattach lost body parts as an action. If he is unable to do so, the body part regenerates. Depending on the size of the body part, it may take minutes or hours.

Hardened Body. In the event that The Nameless One's Mage Armor is dispelled, his armor class becomes 18, due to a +1 natural armor bonus.

Deceased. The Nameless One is the soul of his dead human self. He is therefore immune to effects requiring living targets and his original body can be raised or resurrected as normal (or would be able to, were he not dead from old age). He is considered a human for the purposes of race. His ability scores cannot be reduced. For the purposes of effects that treat creature types differently, he is treated as either a Humanoid or an Undead, whichever is more beneficial to him. He does not age, and does not need to eat, drink, breathe, or sleep.

Memories of Life. The Nameless One adds his proficiency bonus (+9) to any attack roll, ability check, or saving throw that he makes (included in his entry). He adds double his proficiency bonus to all skills (included in his entry), as well as any ability checks he makes with thieves' tools.

No Fate. The Nameless One does not automatically miss attacks on a roll of 1, and he does not automatically hit on a roll of 20.

Magic Weapon. The Nameless One's Mace is magical.

Fighting Style. The Nameless One receives +20 damage to any attacks made with a dagger, axe, pick, claw, or bludgeoning weapon (included in his entry). He also receives +3 to all weapon attacks (also included in his entry).

Stories-Bones-Tell. The Nameless One may speak to the spirit of a corpse, even if they have been turned into an undead creature.

Spellcasting. The Nameless One is a 20th level spellcaster. He uses intelligence for his spellcasting. He has a spell attack bonus of +16, and a save DC of 24. The Nameless One knows all Wizard spells and cantrips, as well as all rituals (from every class). The Nameless One does not need a focus, but must provide material components that have a listed cost. The Nameless One has Mage Armor, Mind Blank, and Foresight already cast before any battle (along with any other spells the Dungeon Master considers appropriate). It is assumed he has regained these spell slots already (see the Arcane Recovery Legendary Action). Foresight gives him advantage on attack rolls, ability checks, and saving throws, while giving enemies disadvantage on attack rolls against him. The number of spell slots The Nameless One has is as follows:

1st Level: 4
2nd Level: 3
3rd Level: 3
4th Level: 3
5th Level: 3
6th Level: 2
7th Level: 2
8th Level: 1
9th Level: 1

Sneak Attack. Once per turn, if The Nameless One hits with a weapon attack, he may inflict an additional 30 (10d6) damage if he has advantage on the attack roll or one of his allies is within 5 ft. of the target and is not incapacitated.

Progression. At the Dungeon Master's discretion, The Nameless One may be improved further by giving him epic boons or alternatives to epic boons, as described in the Dungeon Master's Guide. In addition, he may also be given magic items, or possess custom spells of his own design.

Legendary ActionsThe Nameless One can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The Nameless One regains spent legendary actions at the start of his turn.

Action Surge (Costs 2 Actions). The Nameless One takes an extra action.
Arcane Recovery. Roll a d10. The Nameless One regains a spell slot of the same level as the number shown on the dice, minus 1 (so a 2 allows him to regain a 1st level spell slot, for example). On a roll of 1, The Nameless One regains no spell slots.
Hide. The Nameless One makes a stealth check.

ActionsMultiattack. The Nameless One makes four attacks with his Mace (or whatever weapon he happens to have equipped).

Mace. Melee Weapon Attack. +19 to hit, reach 5, one target
Hit: 30 (1d6+27) bludgeoning damage.

Raise Dead. The Nameless One raises a slain ally within 60 feet. The target is restored to full hit points.
