Nailed to the Sky spell provides guidelines. Vacuum does 1d4 damage per round and target begins to suffocate. Sunlight does 2d6 heat (fire?) damage per round. Alternatively, cold from lack of sunlight, does 2d6 cold damage per round.

It's unclear if characters sufficiently prepared for exposure to space, can use the "holding their breath" rules. In real life, holding your breath in vacuum may cause major damage to the lungs.

I believe someone tried to make a 3.5 adaptation of Spelljammer. So that should have rules and fluff for you, but I can't remember if it was finished or not.

Elder Evils has nice, proper, first party splat book rules on what happens to you if you leave the atmosphere of your planet proper.

So planets on the material plane are in fact floating in a vaccum.

It depends on the setting whether the main planet is in a heliocentric or geocentric system. All settings (sans Ravenloft) are contained within a "Crystal Sphere", which is basically a ball that encases the solar system and only a select things can go in and out. The space between spheres... that's complicated.

There's a select few kids of creatures that live in the Void. The Mooncalf is a good example, there is a whole Dragon Mag article on their ecology.

Where is this established? The only D&D 3.5 information on what's outside of the earth's atmosphere I've been able to find is MotP, which only says that the material plane is a bunch of spheres (planets) hanging in space.

This is the premise of Spelljammer, the 2e "setting" that established that all 1st party setting are technically concurrent. Mentions of it in 3rd edition are few and far between. A throw away paragraph in a splatbook here, a blup in a dragon mag article there, and of course the occasional musing in a web article.

There are three sources that have a bit more. Lords of Madness mentions both the Neogi and and the Illithid spacefaring habits. The Flow of Fresh brains web adventure module partially takes part on an illithid spaceship. Dungeon Mag 92 has the most complete set of Spelljammer stuff for 3.5, presented as it's own isolated setting called Shadow of the Spider Moon or something to that effect, with fluff that doesn't quite match but still.
