Both the vampire and the vampire spawn say "as they did in life". The spawn say they are made by vampires. How is a vampire made?

Wish I imagine. There is a Vampire Lord template and rules to get wish from human sacrifices, the story kind of tells itself. "The Dread Duke sacrificed his wife and child on the altar, then himself. With a whisper his body rose from the stone, leaving his shadow behind."

Note what it says in the Create Spawn ability:


Create Spawn (Su)
A humanoid or monstrous humanoid slain by a vampire’s energy drain rises as a vampire spawn 1d4 days after burial.

If the vampire instead drains the victim’s Constitution to 0 or lower, the victim returns as a spawn if it had 4 or less HD and as a vampire if it had 5 or more HD. In either case, the new vampire or spawn is under the command of the vampire that created it and remains enslaved until its master’s destruction. At any given time a vampire may have enslaved spawn totaling no more than twice its own Hit Dice; any spawn it creates that would exceed this limit are created as free-willed vampires or vampire spawn. A vampire that is enslaved may create and enslave spawn of its own, so a master vampire can control a number of lesser vampires in this fashion. A vampire may voluntarily free an enslaved spawn in order to enslave a new spawn, but once freed, a vampire or vampire spawn cannot be enslaved again.

So a vampire is created via being a spawn of 5+ HD.

If you're asking where the first vampires came from, this varies by setting. In Pathfinder (Golarion), the first ones were called strigoi and came to the Material from the Plane of Shadow (which in Golarion's cosmology, has the NEP at its heart.)

A vampire can also be created with Seed: Animate Dead (http://www.d20srd.org/srd/epic/seeds/animateDead.htm).

Other possible methods:
Flesh of Orcus (Ghostwalk): if living creature ate it and fail subsequent Fort save, they will die and raise as a Vampire (or Monstrous Vampire)
Shadow Sun Ninja (Tome of Battle) - overuse of Balance of Light and Dark
Shrouds of the Unholy (http://archive.wizards.com/dnd/article.asp?x=dnd/dx20021031x) - can create any Undead
Wish or Miracle spell
Ritual of Unlearning or Ritual of Vitality (Savage Species)
Pact with some dark power (like Strahd von Zarovich (https://1d4chan.org/wiki/Strahd_von_Zarovich) did)
Chaos Curse (https://forgottenrealms.fandom.com/wiki/Chaos_Curse) potion (worked this way for Kierkan Rufo (https://forgottenrealms.fandom.com/wiki/Kierkan_Rufo))

A Necromancer Camper one day said to his fellow campers in Necromancy Camp: "you know what would be a fun prank? What if, and hear me out, what if we swapped the black onyx component on "create undead" with bat blood? Dude, Mr.Necros would totally freak out".

...the rest is History. Rumors say that, to this day, they still play pranks on Supervisors in Necromancy Camp, involving drinking all their blood and replacing it with pudding and animating teens who drawned in the lake and giving them Machetes and Hokey Masks. Hillarius Gang they are, aye.
