Hello! I apologize if this is a stupid question, but... how does Sanity work in d20 call of cthulhu?

In the book, it lists sanity losses in terms of fractions, usually expressed as (lower value)/(higher value). For the life of me I can't find where the explanation for the fraction is in the book. Yes, I checked the sanity section.

Anyways, if anyone could give me an insight as to what I'm looking at, that would be cool.

The sanity rules in Cthulhu d20 are mostly the same as the standard CoC rules. Essentially, it's a Will save. If you pass the save, you take the lower value as sanity damage. If you fail the save, you take the higher value in sanity damage.
