Magic items to improved Teleport accuraccy.

Have a character who is going to cap out at 5th level spells, and will have access to teleport, but will never get Greater Teleport, was wondering if there were any magic items that can help improve or negate the risk of the Percentile roll, so I can increase my chances of arriving on target.

The seer 4 power "Anchored Navigation" seems to be a base power you could use for some kind of custom magical/psionic item. It can even be augmented to function cross planes.

Another option is to just planar bind one of the many celestials/fiends that can greater teleport at will, hop into a bag of holding, and have them carry you to your destination.
