So, my understanding is that Careful Spell will only affect Dragon Breath on the turn on which you cast it (i.e. if you use your bonus action to cast it and your action that same turn to use the breath weapon).

How does Empower Spell interact with spells like Dragon Breath or Flaming Sphere? It says "when you roll damage for a spell" - does that mean that a single casting of a spell like this gives multiple opportunities to spend 1 sorcery point to reroll damage dice: once each time you use that spell to successfully deal damage?

Worth noting that the Careful ruling you are referring to is JC SpecialTM rather than being RAW. As for Empower, sure; no limitation on the first damage roll is imposed.

Perhaps, though it does specify "when you cast a spell", so it tracks.

Odd thought on Careful or Sculpt Spell: don't they require you to choose a SPACE that is ignored/gets advantage on the save, not a creature? In that case, wouldn't it mean for dragon's breath that you specify the space, and only when the AoE includes that space would it matter, so it's probably only useful on the first round anyway unless things happen to line up conveniently on future rounds?

Now, for something else on this topic: Would a major image upcast to 6th level and with Distant Spell applied permanently be able to be controlled at twice the normal range from the caster?

Nope.

When you cast a spell that forces other creatures to make a saving throw, you can protect some of those creatures from the spell’s full force. To do so, you spend 1 sorcery point and choose a number of those creatures up to your Charisma modifier (minimum of one creature). A chosen creature automatically succeeds on its saving throw against the spell.

When you cast the spell, you spend 1 sorcery point and choose creatures, not spaces.

Ah, nice. That makes dragon's breath even better.

Yes. You can control the illusion when you're in the spell's range, and if you cast the spell with Distant metamagic, the range is 240' rather than 120'.
