Relatively simple question but not necessarily a simple answer.

If you deal damage to a monster that then Polymorphs into another form- would the damage carry over?

First, answering such questions as this is precisely the purpose of the "Simple RAW for 5e" thread. Second, the simple answer is no. "Hit points lost to [the sword of wounding]'s damage can be regained only through a short or long rest, rather than by regeneration, magic, or any other means." (DMG p.207) However, the target of polymorph doesn't regain hit points, but rather "assumes the hit points of its new form. When it reverts to its normal form, the creature returns to the number of hit points it had before it transformed." (PHB p.266) These normal hit points can be regained only through a rest, etc. The polymorphed hit points are completely separate.
