Found this pic online and started thinking about an elemental fish-man of some sort. A better name would help huh? lol.

earth = gives +4 natural armor? Is there a spell like barkskin that does fish scales instead?
air = freedom of movement?
fire = heat metal: If cast underwater, heat metal deals half damage and boils the surrounding water.
water = water breathing

My idea is to turn him/her into a Scaled Horror class. http://www.theworldofnocturne.com/dndtools/classes/scaled-horror/index.html

Locathahs lack teeth, claws, and other natural weapons, so they are not especially dangerous if unarmed. A weaponless locathah will generally turn and flee. So they are DEF not the base creature.

Sahuagin are the natural enemy of aquatic elves. Sahuagin are savage fighters, asking for and giving no quarter Alignment: Usually lawful evil. All of this plus they don't look "fishy" enough to fit. Color says MOSTLY green but not always so color isn't really an issue. But the other stuff....

Kuo-toa (https://forgottenrealms.fandom.com/wiki/Kuo-toa)?
Skum (https://www.d20srd.org/srd/monsters/skum.htm)?
Vodyanoi (https://baslag.fandom.com/wiki/Vodyanoi)?
Deep Ones (https://lovecraft.fandom.com/wiki/Deep_One)?
Murloc (https://wowpedia.fandom.com/wiki/Murloc)?

Kuo-toa would work nicely. I forgot about them because they aren't in the MM. Thank you :)
The Murloc is definitely something I'm interested in too but I can't find any sort of statistical info.

They are in the Warcraft RPG books iirc, I have a couple on PDF but I’m not sure I’ve their monster manual.

As another option, Anthropomorphic Fish via Aquatic Escape(aka, a refluffed toad).

Similarly an Awakened Fish would actually be pretty potent, as all awakened ECL 0s are, it’s a free 3HD.

Edit: Murloc +2 Dex, +1 Wis, -2 Cha, Medium, 20ft land 30ft swim, Monstrous Humanoid, Aquatic subtype, Darkvision 60ft, Amphibious, Dehydration(must immerse in water 1hr/day if not then take 1d6 damage and a stacking -1 to all rolls every 8hr, this damage cannot be healed til they soak), Swim always class skill, Language: Nerglish Bonus: Common, Dwarven, Nazja, Zandali. Favored class Scout.

Has a racial class, but the Warcraft rules make it optional. It’s a d8 high BaB good Fort/Will 2 rank/lvl class that gives a little natural armor, stealth, better land and swim speed and darkvision.

Further details can be found in World of Warcraft Dark Factions page 10.

An awakened fish? That would be awesome but it wouldn't turn it into a humanoid. Also I don't see any info on any fish to base it off of.
Thanks for the Murloc info too.

Note: Dragon #306/Dragon Compendium has Elemental Demons
Look the picture (https://static.wikia.nocookie.net/sfery/images/7/70/Elemental_demons.jpg/revision/latest?cb=20150207154051)
At the bottom is a Water Demon - I think they look "fishy" enough
Water Demon is 8 HD Medium Outsider (LA +4), and have the Waters of Fire special attack (which boil all the water in 30' radius, dealing 1d8 fire damage and obscuring visibility)

Could maybe be a Hengeyokai (carp) in hybrid form?

That's Slark from Dota 2 (https://dota2.fandom.com/wiki/Slark), looks like your picture is a close-up of the one on this article (https://dotesports.com/dota-2/news/slarks-essence-shift-is-giving-opponents-negative-agility-in-dota-2).

Awakened Fish would just use toad as per Aquatic Escape, similar to various other tiny animals that have stats of "as this other animal with X minor change" like the Needletooth familiar. While the Stormwrack fish could be used, they don't have ECLs like the appendix MMI awakened animals do. The lack of being a humanoid is an issue, but just imagine the hilarity of a goldfish slaughtering people.

The mention of elemental demons reminds me that the Half-Fiend template has options to base the fiend off of any demon/daemon/devil you like. Since it is also a template class, it is easy to abuse onto a character without paying much more than a few thousand exp.

However for ease of use, I would second the Hengeyokai.

Speaking of "Half-..." templates: Half-Dragon (Yu Lung)

Yu Lung is at the lower left:

Also, Half-Dragon (Lantern Sea Serpent) - see Dragon #345

Do you use the half-dragon template that's in the MM or do I refer to Dragon # 345? It looks like it just covers Sea Serpent type dragons in that issue.

The water demon looks very fitting with the "Waters of Fire" ability. My PDF of Dragon Magazine says it's only a 10' radius. Is it a typo by you or Dragon? lol.

I don't have access to the OA book but I managed to find some info online and am I reading it wrong or does a fish have a base land movement of 20? lol

Carp +2 Dex Speed 20 ft., Swim 30 ft.

Half-Dragon (Yu Lung) is in the Races of the Dragon

I meant: use the guidelines from the MM Half-Dragon to make one for Lantern Sea Serpent
(Personally, I never believed in the "every Half-Dragon should be printed separately" nonsense. Guidelines are pretty clear - sans the "multiple BW" cases)
