[3.5] Looking For Magic Items

Hey all,

Trying to find some printed, non-homebrew/non-custom magic items that let you Polymorph/Alter Self/Alternate Form etc. into one specific form.

So, specifically not looking for things that let you assume any form, or chose from a large number of forms. Looking for things that provide one specific form you can change into.

3.5 or non-updated 3.0 please. No PF. Dragon/Dungeon mag is acceptable in this case.

Off the top of my head, cloak of the manta ray gives you a manta ray form in salt water, "as the polymorph spell, except that it allows only manta ray form"

Carnival Mirror ("Mirror, Mirror, On the Wall" article, Dragon #305) - make a save or transform into whatever depicted in the mirror; 28050 gp

Mask of the Tiger ("Dragon Hunters", Dungeon #104) - grants the wearer the ability to polymorph into a tiger 1/day for up to 8 hours; [cursed] item - every time it's used wearer should make DC 15 Fort save vs lycanthropy; 22400 gp

Necklace of Nature's Teeth (Magic of Faerûn) - allow to assume the form (as Wild Shape) of whoever's that teeth is/was; user must be a Druid with access to Wild Shape; 2900 gp

Ring of Dragonshape (Draconomicon) - polymorph into Young Gold or Red (depending on Good/Evil) Dragon for 1 hour; 23000 gp

Ring of the White Wyrm (Frostburn) - one of its powers allow to gain White Half-Dragon template for 1 hour; 64000 gp

Skull of Dragonform ("Wormfood" article, Dragon #342) - transform into a Juvenile Dragon for 1 hour (as polymorph effect, but with Breath Weapon and dragon's sensory abilities); 86000 gp

If Disguise Self is close enough, Dragon 328 has Thespian's Mask.
