How much would this fix the problem of fighters and other melee classes in general becoming second-rate in the later levels: All enemies are Large size or smaller, and the vast, vast majority are humanoids or monstrous humanoids. A few animals and vermin here and there (again, all Large or smaller) are included. Flying enemies are not the norm.

In a campaign setting like this (probably a city campaign), do fighter-types have a lot more options since disarming, tripping, bullrushing, and sundering are suddenly viable?

Especially rangers, whereas those with area spells get nerfed.

Why would they lose their minds?

I am just saying that there is still plenty of "save or..." spells that target individual creatures.

The idea is not bad, but it will only help a little, if at all.

Oh sure casters will still rock, but at least mr chain tripper doesn't feel useless because generic enemy #1 is gargantuan sized with 35 strength.

Rogues could get a decent sneak attack off.

Also it would make grappling checks a lot easier for the PCs, not dealing with trying to pry the mouth of a Purple Worm open and such.
