Contemplate a Wand of Concealment.

It has the usual charges rule for wands. "This wand has 3 charges. While holding it, you can expend 1 charge as an action to: ...[XYZ]. The wand regains 1d3 expended charges daily at dawn."

What it does is to cast an illusion which makes the wielder and up to 4 other Large or smaller creatures appear as something matching the local terrain. The wand is capable of coming up with its own best guesstimate without the wielder needing to give a rationale. E.g, in the desert the group might become saguaro cacti. Underdark, they might be stalagmites.

Mechanics-wise, it does the following: all of the targetted creatures are able to make a Hide check at advantage, and can stay hidden for up to 10 minutes.

What's the proper rarity of the item?

It sounds close to Major Image (3rd) or Hallucinatory terrain (4th) with its effects. Doesn't perfectly map onto either but it's near both. 3rd+4th level spell wands are typically Rare, so that seems appropriate.

Yeah rare seems fine

Staff of the Woodlands is rare and it allows unlimited use of Pass Without Trace plus some other spells, so I would say you can go with uncommon for the wand.
