Of the many ways to get from Point A to Point B, none are so convenient as Portals. These doorways to elsewhere come in many forms, but they tend to be very stationary. However, occasionally, they can be found in forms which allow them to be moved, and very rarely they are intended to be carried.

Portable portals are very expensive to fabricate, both in materials and experience points. The actual game system used for your campaign will affect this, but assume as the item is easier to transport, and links to more distant locations, it's difficulty to manufacture increases by an order of magnitude.

1) Pentagram Rug

The intricate pattern woven into this rug can be activated only once, but it can transport as many beings as can stand upon it. (To be teleported, each occupant must be touching the rug; anyone carried is left behind.)

When activated, the pattern in the rug glows, the glow shoots up and around those standing on it, and the lights fade, revealing only a burnt and smoldering rug.

Those on the rug are portaled to a wizard's pentagram identical to the one on the rug, which is permanently built as part of the floor. Each rug has only one possible destination, though a destination pattern may have multiple rugs associated with it. The absence of traps and alarms depends on the wizard or other being who currently occupies the location of the permanent pattern.

2) Trap Door

This 2' square or round hatch can be laid on any horizontal surface and activated for either of two functions:

A. The hatch opens into a 10'x10' extradimensional chamber illuminated by a dim, diffuse light. Anything placed inside remains until retrieved. Living creatures cannot be smothered, the air replenishes itself, but starvation may be an issue if food and water are not available.
In this mode the hatch may be opened from within if the occupants can reach it so long as the exterior door is lying flat and is able to be opened, (as in a heavy object being placed on it.)

B. When activated, the trap door becomes invisible and virtually undetectable by the usual means. Beings who step on even the edge of the door fall ten feet to the floor of the extradimensional space. There is no access to open the trap door from within in this case, and occupants must wait for those outside who are aware of the trap to open it.

The space accessed by the trap door is the same in either configuration, so even unwilling occupants have full access to the items stored within.

Items and beings must be able to fit into a 2' opening to fall or be placed in the extradimensional space.

3) Magic Mirror Portal
What appears to be an ancient mirror of polished silver, when the right gestures are performed in front of it, transforms into an oval opening between the Material and the Ethereal planes.

The mirror can be placed anywhere on the Prime plane and it's shadowy counterpart will appear in the corresponding location on the Ethereal. One being can pass through at a time, either way, so long as the portal remains open. Moving either mirror will break the link, but it can be reestablished so long as neither is broken. Breaking one mirror destroys both.
