So im gonna play a hexblade whos whole family bloodline has been cursed by some really bad curse. Its like a blood issue of some kind. Anyway i need help to think of a really bad curse that the character is trying to kill their patron over thats not debilitating to the character mechanically or for rp. has to be bad enough hed be willing to give up eldritch blast. also this is my first time on the forum so sorry if im doing something goofy

Most campaigns don't go long enough IC for aging to matter, so any age-related curses could be IC devastating without any mechanical effect in practice. For example:
* Short lifespan, ages twice as fast once reaching adulthood
* Guaranteed to go senile by old age, likely by middle-aged.
* Become old-level decrepit by middle-aged, venerable-level by old age, and your last few years you're completely bed-ridden.

Also, things that are highly unpleasant but happen during downtime or don't impede you in a mechanical way:
* Most nights you have horrible nightmares where demons torture you. Because you're used to this and it doesn't last all night, you're not fatigued, but it still sucks.
* Food tastes disgusting. Water hurts your throat. Clothes always feel uncomfortable. The temperature always feels too hot or cold.

Character can't heal by any means unless the hex weapon has someone else's shed blood in the last 24 hours. Shouldn't be too debilitating for an adventurer, but will drive them to engage fights. Their relatives who had this curse died in battle over this very reason.

A classic one could be "turns into a werepanther sometimes." They have a semi-uncontrollable use of "wild shape into panther."

You have to kill something with a soul regularly. The power level of the soul matters: a farm animal or similar isn't enough oomph, as it lacks sufficient vitae.

Minor magical beasts are better. Humanoids, with their potential to become spellcasters, have lots of vitae in their souls, but are not the only diet.

If you wanted to stop being an adventurer, you'd have to murder a person at least once per month.

When you kill someone you taste their soul being torn apart. It is amazingly vile.

The curse will also take over in case you do something suicidal or fail to feed it.

The souls of all wielders are damned after death (or go to the Outer Realms or a nasty part of the Feywild, depending on the patron) regardless of their alignment in life.

Cannot ever sleep in a space previously slept in.

Anyone he becomes friendly with gets a minor version of the the curse, and anyone he truly loves gets the full curse.

People always assume the worst about him, even when there is clear evidence he's behaving altruistically.

He suffers a magical compulsion to perform a specific disgusting act once a day. Something like eating dirt or worms, or shoving his swordhand into filth. Not debilitating, but definitely something he has to try to hide from the general public.

His sword hand always appears to be covered in blood.

The spirit of every humanoid he slays follows him around, and nobody else can see them, even with TrueSight. They can't interfere with him (usually), but it's distracting, especially if the crowd grows.

Everything tastes like ashes.

Ooh, I like this. But to make it extra curse-y, maybe the flavor depends on the alignment of the person killed. Evil tastes awful (and the more evil the person, the worse they taste), but Good is delicious. And let player discover that part on their own.
