when we first see through the Rift, we see a whole planet from a distance.

later on, we then see what appears to be an ocean.

...what if it's the same planet, getting closer?

Shojo says (https://www.giantitp.com/comics/oots0276.html) the Order of the Scribble found five rifts, and Heimdall also refers (https://www.giantitp.com/comics/oots0998.html) to the rifts in the plura.

I would say that it's common sense. Rifts in different locations on OOTS' world would naturally lead to different locations within the Snarl's prison.

Well, I never thought about it before, but it's general consensus as it's not 100 perfect confirmed that we're looking Into a singe planet.
