Personally I'd just add it in later using The GIMP (http://gimp.org/). Just make a selection around the naughty bits with the height & width being multiples of the pixel size (like 20 x 25 if you're using 5-sized pixels), cut & paste it, then choose the Pixelise filter (it's under blur) and there you have it. Not totally Inkscape, but it's a quick workaround.

I like to draw the naughty bits after I put down the pixelation, personally. It symbolizes how even a blurry censor can't hide the glorious nudity of the human body.
