Don't have the book, but if it looks like Cthulhu, it's likely to be a mindflayer.

That sure as hell looks like one Great Priest of R'lyeh to me. Must be from some forgotten supplement where they had an encounter with him.

That's the picture of Cthulhu from Call of Cthulhu d20. There's a section at the very end with D&D 3.5 stats (not CoC d20 stats; there's a few key differences) for some Great Old Ones and Outer Gods. (The authors decided - very wisely - not to give GOOs and OGs and the like stats in CoC d20. They're completely unnecessary, especially considering that all the BRP stats tend to include some variation of "invulnerable" and "automatically kills a random number of people per turn.")

Well, it's 1d3/round (from his claws) and potentially an automatic 4 from his facial tentacles, if they're in range. If he's fighting something too big to easily swallow, his claws instead do 1d6+damage bonus (of 21d6).
