title question

For the profile title (Halfling in the playground, Barbarian in the playground, etc.) I recently seen a few people with the title "Custom Title in the Playground."

I haven't seen anything on the other threads here about, so I just thought I'd ask, what is this? How do you get to "Custom Title in the Playground"?
