This one is all me.

The adventurers crept slowly through the twisting tunnels. They had already fought off several creatures that had leapt from the darkness, and were somewhat tired and worn. As they slowed to a stop, one of the adventurers noticed a shuffling sound coming from behind the group. Something large and round walked on spindly legs through the darkness, just out of reach. One of the brave adventurers rushed the creature, only to stop in horror. A eye that could have belonged to a giant stared back at him, watching his movements cautiously. Suddenly and without any hesitation, it attacked the man, jutting forward two of its four hooked claws. The adventurer parried its blows, taking only minor scratches. The others surrounded the walking eye, hoping to flank it. But just then, the eye split along an unseeable horizontal line, opening wide. Rows of sharp, jagged teeth lay within its gaping maw. The adventurer had time for only a short cry before the eye leapt and closed its hungry mouth around his head.

Devouring Eyes are large, semi-intelligent eyeballs. They walk on four spindly legs, each tipped with a sharp claw. The eye part of their body conveys visual signals to a specialized brain near the pupil. The do not have any organs for sense of hearing or small. Their body splits open to reveal a large mouth full of sharp teeth. After food enters the mouth, it goes through a complex digestive process that occurs in the same place. The mouth doubles as a sort of stomach. Waste is deposited through a covered hole at the bottom of the eye, usually as a black, sticky ooze. The eyes can eat almost anything, but prefer fresh meat.

Darkvision 60 ft - Unsurprisingly, Devouring Eyes have very good vision. The can see in all but magical darkness for up to 60 feet.

Improved Initiative - Devouring Eyes are fast and surprising hunters. They gain +4 to Initiative checks.

Natural Armor - Because of the great danger to Devouring Eyes of being blinded (giant eyeball), they have developed a thick coating on their outsides that protects them. They gain +3 natural armor to their AC.

Combat: Devouring Eyes prefer to sneak up on their prey in the darkness and then incapacitate it with one quick bite. When spotted, they switch to using their more dangerous claws to wear down the enemy before attempting to eat it.

It's not too bad but you've made a few mistakes.

A 2 hd monster doesn't qualify for 2 feats. It doesn't need Improved Natural Armor because you didn't give it a natural armor bonus in the first place. Simply give it +1 natural armor and leave it at that. Also, it qualifies for a size bonus to AC for size +1 too.

You left off the bonus hit point +4 (and it should have 13 HP not 12, 4.5 and 4.5 and 4 = 13.

Without multiattack Feat, the creature's secondary weapon (even if it is a natural weapon) is at -5.

Darkvision should be Darkvision 60 ft.

Advancement should be 2-4 HD (Small) and generally monsters triple their HD when they advance.

Grapple is Base attack bonus + Strength modifier + special size modifier (small creatures are at -4 for their size) so you get 1+1-4 for -2 grapple.

Melee attacks are : Base attack bonus + Strength modifier + size modifier
+1 +1 +1= +3 not plus 2.

This isn't a difficult monster for a party of 4 -- CH is probably 1.

It has 5 skill points so you can give it one more to Spot.

LA should be — as it is not suitable as a Player Character Race.

I've made the changes Here:
