I was looking at friends profile, I noticed he wasn't part of any public groups and then I wondered how do you start a public group?

I believe (stress on believe) they have to be set up by the site admin or someone with access to the database. So the short answer would be, you can't.

That would be post counts, fight club and user titles. There simply is no such thing as a public group as we never created any.
