Real-world diseases

I'm thinking of creating a post-apocalyptic d20 Modern campaign setting, in which something like getting pneumonia would be nearly as big a threat as combat. However, I can't find effects for any real-world diseases statted out anywhere. Is there any sourcebook or homebrew that gives effects for real-world diseases?
