Considering that Touhou is a pink bishoujo ghetto, any character of importance will be female, with the exception of Genji and Rinnosuke. Not sure if Unzan is present on an individual basis rather than represented by his hitbox. I'm also not sure if the orb from Highly Responsive to Prayers counts either.

Otherwise, Wriggle is the closest thing to a male in Gensokyo due to her style of dress.
