I was always a big fan of the whole 'X stat to Y bonus thing', but I wanted to point something out real quick.
While looking through what you have here it was kind of hard to differentiate between the source of the ability and what the ability did. Maybe underlining or italicizing would be handy.
Example:
Illumian (Uurkrau) Bonus Spells RD p54 3.5
or
Illumian (Uurkrau) Bonus Spells RD p54 3.5
or
Illumian (Uurkrau) Bonus Spells RD p54 3.5
Feel free to ignore me, just pointing out the first thing that came to my mind :)

Different organization from what I recall, and GitP is a really weird place to choose to place optimization information but I do appreciate the work. WotC forums are so screwed I've been sticking to using the Google caches.

I think that a table format is going to be too large, and you would need separate tables for each subheading under each stat to keep it organized. Probably the best would be using [code] to format it, like this reposting at Brilliant Gameologists.

Feats and Classes aren't separated in your Dexterity list.
- Chameleon Base Class [3.5]/[PF]: A versatile, morphic class that mimics one basic party role (warrior, caster, sneak, etc) at a time. If you find yourself getting bored of any class you play too long, the Chameleon is for you!
- Warlock Power Sources [3.5]: Making Hellfire Warlock part of the base class and providing other similar options for Warlocks whose powers don't come from devils.

Actually, looking at the example with the tables above and the code below, I found the tables to be much more legible. Empty space makes it easier for me to parse.

Dead Eye only works with ranged weapons (in which you have Weapon Focus), and only as a point-blank shot. The ranged weapon limitation, at least, should be noted.

The Alternate Class Features from Drow of the Underdark are not substitution levels, or even exclusive to drow, despite some of their names. And the actual ACF name is Hit-and-Run Tactics, not Drow Fighter.
