Thri-kreen is the standard one. Four arms, 2hd monstrous humanoid, LA +2 for the psionic version and +1 for nonpsionic. They look kinda like mantis-men.

Psionic version is in Savage Species/XPH. Nonpsionic is in MM2, I think.

Edit2: If you have the Dragonblood subtype you can take 'Draconic Tail' and then 'Prehensile Tail' for effectively another hand. A dragonblooded thri-kreen might be a hard sell - you'd probably need the Draconic template at +1 LA - but with that and multiattack you're up to quintuple-wielding.

I'm not sure if the Yuan-ti graft 'extra tail' qualifies you for Prehensile Tail. It gives you a constriction attack, but I suspect that doesn't count.

Dolgrims 1HD, 2 LA. 4 Arms, 2 mouths. They can make offhand penalties that are one handed at no penalty (so don't need light weapons for TWfing to be -2/-2).
DR 5/byeshk so very few bypass it.
