    Becuse monsters with size of small and higher can still come in enormous quantities.

    Those would be 'mobs', and the rules are in the DMG2, pp. 59-61.

    Go to the homebrew and looks up the special mob rules someone statted up there. They are fantastic (but a little OP, so aim for 2 CR below normal).

    They also made their own rules for mobs in the World's Largest Dungeon book. To list another book you don't have.

    Wait where are you getting swarm rules in the first place? PHB shouldn't have them i don't think.

    The rules for mobs are also in the appendices of the Shackled City Adventure Path.

    You're right. They're in the MM, under both the Swarm entry and the Swarm subtype at the back of the book. I'm fairly sure it doesn't give rules for making your own or judging CR of those you do create. This is probably a good thing when I was planning to make a Stirge swarm and was wondering just how much Con damage it would deal each round.
