The most times you can get an extra attack for -2 to hit as far as I know would be to dual wield eagle claws with eagle's fury from Sandstorm, be a monk and make any weapon fury-able and attack with flashing sun and snap kick. -10 to hit for 5 extra attack.

Flashing Sun allows a normal full attack routine, so yes, that should work with TWF, ImpTWF, and GrTWF. Snap Kick should work, too, although that's a -4 attack penalty at that point (you can bring it down to -2 with two levels of Bloodclaw Master). Add a few levels of Totemist for another half-dozen or so secondary natural attacks.

An Eternal Blade 10 could also do this twice in a round with Sudden Recovery + Island in Time, although the usual trick is Time Stands Still + Island in Time.

You might also be able to add some shield bash attacks on top of that with a little Ruby Knight Vindicator, Shield Counter as an immediate action, and Divine Impetus to recover/convert your turn attempts into more Shield Counter attacks. I haven't quite worked out all the mechanics of that, though... and RKV 7 doesn't quite fit with Eternal Blade 10.

If you really wanna get ugly, throw in 4 levels of Thrall of Demogorgon for Dual Actions.
