Anyone good with the vestige tablets?

Hello, all. Currently I am working on a homebrew project over in the rpg forums, and part of it involves creating a vestige. It seems that I suck at making the tablet things.

For those who don't know what I am talking about, all vestiges have a picture like one of the following:



Basically, they have the vestige's name on top, followed by the seal, and in the bottom part all vital information. Perhaps someone could take a whack at it?

I've already got a template:

The seal is supposed to look like the spinosaurus skeleton in the Jurassic Park III logo. It is also provided below.

In the area for the vital information, I need the following written

Vestige Level: 7
Binding DC: 32
Special Requirement: Yes

As for the name, its "Azuvidexus, the Ravenous Maw"

If anyone would do it, I'd be eternally grateful.
