Karsus' avatar certainly did, but I don't remember just killing a deity did that...

(Anyone who cares enough to stat me up can, so long as they don't do it in a way which is obviously meant to be offensive).

Nitpick: Karsus's avatar didn't kill the god, it allowed the user to command their power for a limited amount of time. Karsus was unable to sustain the weave however, and Mystrl sacrificed herself to sever his connection to it so the damage wouldn't become irreparable.

Karsus's intention was to borrow Mystrl's powers so he could save Netheril, not become a deity.

Afroakuma started off your average run of the mill incarnation of all evil in the universe. However, due to fate, random chance, or a whim, he was born with something unique: An afro that acted as an extension of his brain. Even as he traveled the multiverse spreading evil, he remembered everything that he encountered, and soon grew to become one of the greatest sages of lore in the universe. As his knowledge grew, so did his power and evil. After a time, he grew bored of learning all the information of the universe; he already had it. When this occurred, the works that would launch him into deityhood began.

Over aeons, Afroakuma conscripted mortals into projects to create entirely new universes, with new sets of rules, participating in the excavation and renovation of the crystal sphere of Kamala. After following through with its completion, he felt invigorated, and began work to create an entirely new crystal sphere, a feat never before accomplished by any save an overdeity. He named this new, primordial place Perantea, and began to plan and begin its design. However, infernal politics prevented him from properly forming the sphere, though enough was accomplished through his work that he began to ascend to divinity.

After founding a cult of planar scholars of all alignments on the mortal realm, his power grew great enough to become a proper God, and his influence and the diligence and public works of his priests and those that served under him caused him to quickly become an Intermediate deity, a position he holds to this day.

Those who attempt to contact afroakuma with Contact Other Plane about Planar matters may treat him as a Greater Deity, though there is no chance of a random or 'don't know' result. Prayers are recited in the form of question about the nature of the multiverse, whacking themselves on the head, then reciting the answer. The clergy of afroakuma tend to be either very cynical or very inquisitive, leading to an odd blend of individuals in his cult, which maintains a moderate open presence in metropolises around the multiverse, primarily on Sigil.

Doc_Maynot comes from humble begins, attempting to maximize her ability to heal her comrades, and even attains great renown for her unique efforts. Shortly after achieving this goal, Doc_Maynot tuned toward the study of Lycans and became enamored with them through one of Doc’s many questing partners, gaining great insight and knowledge into their kind. With this new knowledge, Doc began gaining renown with the deities of the Playground and conversing with them much more regularly, going so far as to master her rage and love of music, channeling her primal instincts to grant herself and her allies far more prestige within the world than before. Due to Doc_Maynot’s efforts she has ascended beyond the mortal she once was, and now grants her blessings to any aspiring Healers, Bardbarians, and natural or newly created Lycans, whom she has lovingly dubbed her Mooninites. From her large pool of abilities, Doc has begun to not focus on her healing or rage buffing, but to become a generalist so that her variety of worshipers may better get to know her. Some say she has started to camouflage herself within her clergy to find those most worthy of her teachings and leads them on a new path, one with many varied abilities that change to fit the needs of their allies and families.
