    This is a solo campaign using D&D 3.5, lots of homebrew, and DM overrides.

    In the back partition of an operating room off the side of a small hut built into a hill at the intersection of a plain, a jungle, and a desert, Cain stands hunched over the corpse of a hound laying on a lacquered wood operating table. A fore leg, a hind leg, both ears, and its tail have been completely removed and set aside. He is digging about in the deceased creatures abdomen looking for any parts that may be of use later when he is interrupted by a hurried pounding on the outside of the hut.
    A voice sounds in Cain's head, "That's probably the owners of this dog you killed. They probably want it back."

    Cain proceeded to take off his gloves and put them in a nearby basin filled with water that smelled of various herbs. The water was hot, heated by the fire underneath it. He had many gloves, but buying new ones was always a bother. This is why he was sterilizing those that weren't too worn down. A simple process but still seemed like it could be more. 'Damn those wizards and their cleaning spells' he thought as he approached another basin. He turned the tap on, letting the water start flowing, before proceeding to wash his hands, using a soap he made through a mix of his own skills, herbs he found and the soap bought from a store. Gaining access to the nearby spring, along with properly doing the plumbing required a lot of effort and some help from the local plumber when he screwed it up, but it was worth it.

    "I didn't see the owners anywhere and the dog went towards me. I think he was simply oferring himself to help me further the research." he said. Moments afterwards he turned the tap off and wiped his hands on a nearby white towel. Black and white, a good color scheme that was accomplished very easily. Where others used steel or any durable material, he had his resin. His beautiful, oh so useful resin. As strong as steel, looking like the sky at night and self-repairing. It also did not irritate his skin the way the metals usually did. He took of his surgical mask along with his white coat he so adored to wear when performing procedures, hanging them on a nearby coat hanger, before exiting his medicine room.

    Cain's footstepps were muffled by the fluffy white carpet that covered the floor of his living room, which was also the place where he greeted his patients. It was meant to be cozy and a comfortable place to sit when he was resting. A worn down but still comfortable to sit and sleep on couch, a few armchairs and a small table. There were lamps here, which he didn't need much personally, and also various little things he found pleasing to look at, along with two chests that stored various items, equipment and such. There was even a small desk to the side with a chair, a few inkvials and a pen. There was even a small bureau to store cutlery and some clothes in other drawers. Nothing too expensive but well made. His resin couldn't replace anything but he could damn try, which made the amount of money he had to spent to survive... rather low. Especially when he could catch food on his own, though he did like a meal in town's inn once in a while.

    He shook his head and proceeded to open the door, looking at his, hopefully, future patient.

    "Welcome to Cifer Clinic. What is the nature of your injury slash disease slash problem?"

    The voice seems to ignore Cain's remark and continues on, "Now you're going to have to explain to some little girl that she's never going to see her dog whol-" The voice stops when Cain sees what is outside. A burly half-orc is standing outside carrying a slightly overweight dwarf who has several arrows protruding from his chest and a very slowly expanding circle of blood around each one. The arrows themselves seem to be staunching much of the blood loss that would be happening if someone tried to take them out.
    The half-orc waits impatiently for you to finish your introduction and then for the dwarf to mutter some incoherent response before speaking in a panicked tone somewhat higher than you expected from him, "Doctor, my buddy and I were out and about and we were attacked. He got it the worst. Do you think you can fix it?"

    Cain looked closer at the injured dwarf. Arrows, really? His eyes went back to the half orc, as he himself remained calm, although inside he was a happy. How could he not be? After all, it was time for operation.

    "Bring him inside" he said "and don't worry. I will make him all better~!"

    With that he walked back into the house, going towards his Medical Room as the half orc followed him. In his mind he was already making plans. The weak skin at the chest simply couldn't stop the arrows, so he will give him a tougher one, but later. Right now he had to check the damage to internal organs. If the heart was struck, the dwarf wouldn't have lasted this long, but weirder things happened. If the lungs were damaged, he would improve dwarf's respiration. Why did the oxygen have to be provided by lungs and not skin? Ah, so many choices. He motioned for the half orc to put the Dwarf on operating table as he donned a clean pair of gloves and surgical mask, while also bringing some means of anesthesia, along with Svizzera, scalpels, needle and thread. He began by cutting away clothing near the wounds to not aggravate them further by removing shirt the usual way... And proceeded to assess the damage.

    "You can wait in the living room if you'd like." He said to the Half Orc

    The half-orc leaves the room while rubbing his hands together nervously and looking back.

    By the position and the angle of the arrows it looks like the heart survived unscathed. However it seems that the left lung, the stomach, and possibly parts of the intestinal tract sustained serious damage. Fortunately the rest of the arrows missed anything major and will just need removed and the wounds cleaned and stitched up.

    The voice pipes up again, "I know you're considering it, so don't. Just clean up the wounds and make him like he was before he was shot. Neither he nor the half-orc asked for it."

    Cain said nothing as he applied anesthesia. He was deep in thought.

    "Then a small improvement. Make him more likely to survive next arrows" he said. He went towards his "storage". The place where he kept his fresh harvested tissues and such. He will make the stomach and intestines what they were before, but the lungs... Yes, less easy to puncture, more sturdy. Better in the gas exchange process, making it more efficient and keeping the dwarf with slightly higher amount of oxygen. Warriors and the like tend to forget about breathing properly and get tired more easily. This one will take more than that to gasp for breath.

    He began the operation.

    (2 points to replace ruined stomach and intestine, 3 for improved lungs. 5 total used up)

    A series of arrows are removed from the flesh, the ruined organs removed, and the new organs sutured into place. Cain ensures that nothing was left behind in the wounds and stitches everything up.
    All that's left is to wait for the dwarf to come out of sedation and for him to heal over time. That new lung should hardly be noticeable, his skin will grey slightly as it comes to full capacity but he should be able to take a bit more a beating once it's over.

    The voice sighs, "At least it won't stand out too much, I'm taking that as sort of a victory."

    the dwarf begins stirring from his drug induced slumber.

    Cain nods and takes a step back. If the Dwarf was barely conscious when he was brought in, he might react a bit violently to his unrecognizable environment. He calls out towards the Half Orc.

    "Operation complete. He will recover, better than ever."

    He would wait with mentioning payment until he sees half orc's reaction, along with that of the dwarf.

    The half-orc stands up from the chair he was in and slowly walks into the room and over to the dwarf. He looks him over and breathes a sigh of relief. He turns to Cain and says, "I can't thank you enough." He reaches behind him and pulls out a small coin pouch and hands it to Cain. "Will this be enough to cover it?" The pouch contains a few gold, several silver, and a small handful of copper.

    Don't count them right now, that'd be rude, and it's definitely enough.

    As Cain looks in the pouch the dwarf suddenly gasps for breath. He sits up part of the way quickly, puts an arm around his midriff with a grimace, and sit up the rest of the way more slowly. The half-orc dashes over to him. "Are you okay?"
    The dwarf feels his torso and, seeming to decide that he was actually alive, takes a deep breath, "I'm alright. I thought I was going to die," he looks at Cain, "Thank you so much."

    Waves it off. "I was just doing my job. I think you might be happy to hear that your lungs will actually be better than they were before. I assumed you might need it, if you two once again have to dodge arrows." He put the coin pouch in his pocket as he went to put the gloves in sterilization basin.

    "If there is a story that comes with it, I'd be interested in hearing it. Unless it is a tale you don't want to share?" He asked. It was true, he was interested in the tale of this but if they didn't want to share, then he could deal with it. He started washing his hands in the meantime.

    The two look at each other for a moment before nodding slightly. The dwarf turns and says, "We were coming home from the docks when we got mugged, we knocked the fellow out but it seems he had some friends watching from the roofs to make sure nothing went wrong. Once he went down they started firing and we started running."

    He simply took off his surgical mask and shrugged.

    "Then I made a good choice by deciding to not live in the town. Was the surgery all that you needed?"

    There was not much he could do. It was not his problem. Although...

    "Remember, if at any point you need to be stitched back together, or require some sort of improvement, you know where to find me..."

    He mentally sighed. Oh, how he wished to have an assistant or someone to help him. Those surgeries were a way to not only get profit but also experiment slightly, depending on what needed to be fixed. However, as he lacked any way to quickly return to his house if an emergency arises, he couldn't venture far out to get anything new for his research. Any life saved here was a life that would rely more on him and allow him to do more, ruining this "relationship" would be bad.

    The half-orc shakes his head, "No, I think that's it." He helps the dwarf off the operating table and out of the building. The door closes behind them and just like that, they are gone.

    He just shrugged and first checked how much money exactly was in the pouch given to him, before going to clean the operating table. After that, he got rid of the unneeded remains of the hound he was dissecting earlier.

    Now, he was considering his options... He could always leave and try to catch some animal, to further his research. Why not? That was what he would do.

    After counting the coins Cain proceeds to remove what bits of edible flesh from the dead hound that are left and then dumps the remains into his furnace.

    Cain simply took the edible pieces of meat with him, on a resin tray of sorts, into the Kitchen... And started making himself a dinner. Meat is meat, after all. Whether it's from a hound or a cow, it doesn't matter.

    After preparing the meal, the process of which intricacies I won't reveal here, he cleaned the dishes and went towards the front door. His white coat was replaced with a black cloak. His hand rested on the doorknob. If noone were to knock on his door now, he would go and try to find another source of flesh for his surgeries. Sapient donors were rare, especially since recovering from it would be... rather hard.

    Cain thought it over. He was unprepared for desert and the plains seemed... Too plain for now. So with that in mind, he went to the jungle.

    The moment he set his foot past the first trees, his form seemed to shift. The red irises of his eyes expanded as the eye-shaped pupils increased in size accordingly, sometimes blinking as if they had eyelids of their own. The white strands of his hair sprouted black dots. These dots contracted and expanded in response to light, moving as far as end of the white strands they were located in, like actual pupils in whites of eyes. His whole head being covered in such hair-eyes, he kept walking. His vision sharpened and provided him with better grasp of situation. He needed no light either, his vision seeing without it, though black and white. He stalked through the jungle, looking for his "donors"

    After trudging through the undergrowth Cain spots some sort of creature lurking just on the edge of his darkvision. It's hard to tell but it looks to be some sort of wild cat.

    He took out one of his scalpels. In his hands, it was also a perfect weapon. He approached the beast slowly, as his body returned to normal. However, in his mind eye it shifted once more. Less dark vision, more defense. Better reaction time so he could dodge with more ease. Instead of many eyes, simply one more, vertical like his pupils but on his forehead. Then he returned to his released form...

    He pointed his palm of right hand at the animal and concentrated, a small hole opening up in it with a weird squelch like sound, before a glob of his resin shot at the beast, hopefully trapping it there.

    (Lose Many and 2 Dark visions, give me Efficient Eye instead.

    An orb of a sticky substance arcs toward the creature. It attempts to leap out of the path of the shot, but the resin still strikes the creature's hindquarters. A meow of surprise followed by a sharp hiss follows the creature as it falls to the forest floor. It sounds as if the creature is continuing to struggle against the substance in the underbrush.

    He moved over towards the creature, making it look like he took long graceful steps. He twirled the scalpel in his right hand. He had a chance to actually end this quick and without drawn out fight, he concentrated, using his amazing sight in order to locate his target... and then cut the creature's jugular vein.

    Cain finds the creature to be a leopard. Cain slashes towards its jugular in an attempt to end its life swiftly but the leopard jerks away before the attack can make contact. With a snarl the beast retaliates with a claw but falls short.

    He simply narrowed his eyes and attempted yet again to cut the leopard in its jugular. If he doesn't finish it while it is still unable to defend itself properly, the fight will become... quite hard.

    Again, Cain slashes in an attempt to kill the creature quickly but as before the leopard manages to move out of the way before the strike hits. It attempts to struggle from the resin it is entwined in but can't seem to free its legs.

    After a short while of the leopard evading attacks and failing to remove himself from the resin, Cain finally manages to land several blows. The creature continues to struggle against his bonds as he bleeds out. His actions slow as his life flows from his body until they stop completely.
