    Round 1

    Jan casts her spell, the magic from her bloodline causing Frederick to grow to the same size as their enemy. Frederick moves to the edge of the stinking cloud, its magic keeping it enclosed and him safe. He's close enough to get a faint hint of it, a smell somewhere between rotting eggs and vomit, making him keenly aware of the lizardfolk king's fortitude to be inside it without effect. Olofire waits at another edge of the mist, looking intently for the foe but seeing nothing in the greenish fog. Amearin tosses a bolt of frost into the cloud before moving around to get cover from the heavy roots and earth near him. Lydia also moves to the edge of the cloud, her spear at the ready.

    "More spells, huh? How droll...Now, who will come and play with me in here?" taunts Shukak as he launches his trident, this time missing Jan as it whisks by her before returning to the fog.

    The map is updated. Begin round 2!

    Jan flinches involuntarily as the trident suddenly pierces the veil of fumes to head more or less in her direction... but as it fails even to impact one of her illusory doubles, the gnome lets out a peal of laughter. "It seems that the gas is affecting your aim, your wits having already been too addled for a difference to be clear!" taunts the sorceress. Nonetheless, she takes a few steps toward Lydia, clearing her throat and gesturing the cleric to draw close...

    The trident keeps flying in and out of the smoke thankfully without purchase. His allies seem to be floundering..

    He takes a deep breath and opens up one of his extracts before making his move.

    (I read that link, DarkOne, but I'm getting the opposite interpretation as you. By my reading, Amearin would be able to target the center of the cloud with an area effect like create pit. I don't think it matters much for this combat - just want to make sure I'm not missing something obvious!)
    A line of effect is a straight, unblocked path that indicates what a spell can affect. A line of effect is canceled by a solid barrier. It’s like line of sight for ranged weapons, except that it’s not blocked by fog, darkness, and other factors that limit normal sight.

    You must have a clear line of effect to any target that you cast a spell on or to any space in which you wish to create an effect. You must have a clear line of effect to the point of origin of any spell you cast.

    Frederick growls quietly as the chieftain makes attacks at random. Two can play that game... He looses his offhand grip on the falchion and bends down to retrieve his dagger. As he rises, he flings it forward, largely guessing where the chieftain's throne was.

    As she taunts the lizardfolk king, Jan casts another spell to enlarge an ally, this time Lydia. For her part, the cleric grows proportionally to both Shukak and Frederick and readies an attack. Frederick throws a dagger, eliciting no response from the enemy. Olofire, however, decides to take a drink and then take the plunge, heading into the cloud to try and find their foe. His coughing demonstrates to his allies his misfortune - he has succumbed to the noxious vapors!

    "Bah! I tire of this!" grunts Shukak as he barrels out of the fog, right between Frederick and Lydia, his fury aimed at Jan! His piercing strike with the magical trident unerringly (and unluckily for Jan) strikes true in spite of her mirror images, dealing 24 damage to the little gnome!

    As he appears, both Frederick and Lydia strike (Lydia twice!), with Frederick's cutting particularly deep. The hiss of pain from the monstrous reptile makes it clear that the blows have bloodied him.

    The king cries out in the sibilant tongue of the lizardfolk and dragons, his pain and hatred showing. As he does, five ink-black kobolds, armed with rapiers and hand crossbows, leap out of hiding places throughout the room!

In the suddenly close quarters she tosses her spear aside and draws her longsword. Brandishing it in both hands she lays into the chieftain opposite Frederick. "For Heironeous!!!"

    ****.. ****.. ****! I thought that cloud smelt funny! Olofire can only cough as he tries to clear his lungs and runs deeper into the could hoping to hide in the cover till it subsides. Eventualy he comes to a small pool of water and tries to wash it away.

    Jan lets out a gasp that is equal parts pain and outrage at the chieftain's successful attempt to penetrate her defenses. Her face contorted into a grimace of hatred, she steps just out of the giant lizard-man's reach. With a few hushed words and an angry scooping gesture, a pit suddenly yawns beneath the reptilian foe. "Since you can't play nice," she calls out mockingly, "I think you need a time out!".

    Round 3

    Both Frederick and Lydia land hits on the lizardfolk king, leaving him bleeding visibly from their attacks. Olofire attempts to clear his lungs while inside the cloud, but is unable to do so - the water does not help. Both Amearin and Jan get clear of melee and conjure extra-planar pits, one targeting a kobold and the other targeting Shukak. The kobold responds quickly, grabbing hold of some roots with one hand to hold his wiry form out of the hole. Shukak and Lydia leaps backwards, the lizardfolk king into the Stinking Fog cloud where he disappears and Lydia backwards away from the pit.

    The newly appearing kobolds attack, mostly with their hand crossbows, as they feel out their enemies' defenses. One goes to the edge of the fog where it finds Olofire, stabbing at him with its rapier (and missing). The next barely misses Lydia with a crossbow bolt. The one hanging over the pit misses Amearin. The one attacking Jan hits one of her mirror images, and the last one misses Frederick after stepping away from the oversized half-orc. From inside the fog cloud can be heard a kobold's barking cough, as it is apparently effected by the cloud's foul smell. Shukak is silent this round, apparently not foolish enough to give away his position to his opponents.

    Upon being dropped, Lydia's spear returns to normal size, in case it matters later.

    Amearin and the enlarged Lydia share a brief glance, and an unspoken understanding seems to pass between them. Suddenly they shift into action in opposite directions, each attempting the same tactic!

    Lydia thrusts her sword skyward and calls out, "Servant of Light, lend me your aid!" Behind her in the midst of the kobolds, a being of pure light materializes! It hangs gracefully in midair, and the holy light emanating from it bolsters the party's spirits even as it strikes fear into the hearts of the kobolds. Suddenly from within the orb of light, two smaller rays shoot out towards one of the kobolds!

    If Amearin isn't interrupted, I believe the centipede will appear and get to act right before his next turn, and act on his initiative from then on. For his "just summoned" action the centipede will chase after the nearest kobold and try to bite it:

Casting a quick look over her shoulder at Amearin, caught up in the effort of spell casting, the gnome grits her teeth and turns a menacing expression toward the smaller reptilian menaces that sprang from the shadows. She activates the wand of Shield in her hand before dropping it, hastily readying a tiny dagger as a subtle distortion appears in the air before her and her illusory doubles. "Ha! Cowards, the lot of you, sneaking in shadows and hiding from your betters! Come and get me, if you can!!"

    Olofire moves deeper into the fog, trying to avoid the kobold, and instead runs into Shukak! The lizardfolk king is tossing aside a potion and seems as surprised to see the halfling as Olofire is to see him (no AoO due to full concealment prior to being adjacent). "He’s [COUGH] over here! [Cough hack]" cries the halfling to his allies, outside the noxious cloud.

    Lydia invokes the power of Heironeous and summons an angelic ally, which zaps one of the nearby kobolds with a ray of light. She then moves to be in position to harry another kobold. Amearin steps north and begins casting his own spell of summoning, which will conclude in the following round. Frederick moves to attack the nearest kobold, attacking around the corner to maximize his own defenses against the newcomer. Jan readies herself for melee, dropping her wand and readying a dagger.

    "Rawr!" screams Shukak as he attempts to savage Olofire. The king stabs at him twice with his trident before biting and clawing too! The attacks miss, some due to the fog and others due to his unhinged fury and Olofire's desperate dodging, and the king steps away into the fog, leaving the nauseated halfling alone for the moment.

    The kobolds begin to swarm about, mostly firing more crossbow bolts at the archon and Lydia. One kobold stabs Lydia (with a thrust of its rapier for 3 damage) and another hits the summoned archon with a crossbow bolt (for 1 damage). The kobold hanging onto the vines above Jan's pit falls with a muffled grunt. Finally, one more kobold emerges from the fog, coughing violently, behind Frederick.

    "You dare strike a servant of the Archpaladin?!" Lydia roars as she presses in close on the kobold that wounded her. She counterattacks with an enlarged blade almost as large as it is! Across the room, the lantern archon continues floating and glowing in eerie, majestic silence. Two more rays of blinding light streak out from its center towards kobold that damaged it.

    Frederick grimaces at the pit between him and his quarry until another Kobold appears behind him. He turns to attack, when Amearin's call reaches his ears. "You're next, then," the unreasonably large Half-Orc mutters, a rumble promising violence to the newcomer, in due time. Instead, Frederick takes a breath to pause and focuses on the cloud. As soon as he can see enough of the likewise-enlarged reptile to focus his efforts, he charges forward with a roar!

    At Amearin's warning, Jan likewise pauses, turning from her wary watchfulness at the kobold minions to watch for the clouds to dissipate enough to target the hulking lizard chieftain. With a murmur and a gesture reminiscent of a scullery maid emptying a pot of water, she conjures into being a translucent slick beneath his feet! "Some days sure take it out of you! Why not have a lie down, pal?" she calls out mockingly.

    Lydia steps towards the nearest kobold and misses while her summoned Lantern Archon disables the kobold that struck it. Amearin's Stinking Cloud expires, revealing Shukak between Olofire and Frederick. The elf misses with his Ray of Frost and his summoned centipede misses with its attack. Olofire finishes recovering from the nausea effect, the suddenly fresh air welcome in his wracked lungs. Jan casts Grease underneath Shukak, causing the lizardfolk king to slip and fall. Then Frederick steps towards the prone enemy and strikes him with an overhand slice of his sword.

    Frederick hears Shukak angrily mutter from the floor of the chamber,"Fine! If I must die, I will take some of you with me..." before the lizardfolk king cries out, "By the blessings of Kyuss, I take Ilthane's gift!" He then drinks a potion (taking another stabbing strike from the paladin) as it seems the very sunlight dims...

    A moment later, the six kobolds cry out in pain as they each burst in an explosion of green worms, leaving behind small swarms of the segmented creatures that instinctively begin to crawl towards the nearest living creature. As the reptiles die, the PCs can almost see their very souls fly across the room, entering Shukak and transforming him. Pitch black wings rip out of his back and his scales begins to crawl with little green worms. Though not mechanically any larger, the king seems invigorated and suddenly larger in presence, if not size. "Worms will feast on your corpses!" he bellows as he rolls and takes wing, ready to conclude his battle with the party!

    Phase 2! The map is updated. Begin round 6!

    Suddenly uncertain, Lydia takes a step away from the writhing mass of green worms where two kobolds once stood. "What is this?! By the gods, Shukak, what have you done?!" Rushing towards where the lizardfolk chieftain is hovering overhead, she swipes up at him with her longsword.

    The lantern archon's brilliant light returns, having been momentarily dimmed by the lizard chieftain's transformation. A pulsating hum can be heard from its core as two more rays of light streak out, blasting away at the nearest mass of writhing worms.

    "Where is it?!?! Not it, not it, not it..." Amearin frantically flips through his collection of scrolls, looking for a particular one... "Aha, got it! Hold on guys, backup is coming!" He takes a step towards Lydia and Frederick as he begins casting a spell.

    Meanwhile, the summoned centipede's mandibles are suddenly lit from within by holy radiance, like a pale reflection of the lantern archon's silvery light. It lunges forward with supernatural ferocity, chomping at the worms before it.

    Amearin will 5ft step to M/20 and then begin casting Summon Swarm from a scroll. He's going to try and summon a spider swarm on top of the worm swarm in F/19, assuming he's not interrupted.

    The taunting, upward curl of Jan's lip begins to droop, until her jaw drops at the grotesque wrongness of the worms bursting out of the kobolds' bodies, even those who were (or seemed, at least) healthy and very much alive. Once again beset on all sides, the gnome barely has time to engage with the suddenly airborne Shukak.

    Turning to the corner where the summoned centipede engages in an apparently fruitless effort to thin one swarm of worms, the gnome sorceress murmurs a few words, her (and her several remaining illusory images) make a scooping gesture, and another pit yawns beneath one of the swarms. "Hopefully that'll slow it down..."

    At last the fog lifts and Olofire's cough dissipates, just in time for him to see the terrible transformation of Shukak and the dissolving of the kobolds (wait, kobolds?!) into a mass of worms!! If things couldn't get any worse, Shukak begins to take flight and Olofire turns his attentions to a problem he can more easily assist his team with.

    He reaches into his pack for a new flask, one that is warm to the touch and almost glows and orange-reddish hue. He moves away from Shukak and as soon as he is within range, tosses the flask into the mass of worms closest to him!

    Lydia moves and strikes Shukak with her longsword. Her summoned archon says, "I'm afraid I cannot help you with the swarms, noble heroes, but I can help you with that monstrosity!" as it drifts towards Shukak (to try and place him in range of its aura) and hits him with one of it's two rays. Amearin begins reading from a scoll again, this time to summon a swarm of his own to fight the worms. His summoned celestial centipede, lacking an attack that can really injure the swarm of green worms instead rolls on them violently, attempting to squash as many as it can. The attack appears to be a little successful, but only barely (half damage, allowed due to the smite evil use). Frederick misses with his attack and Jan drops the nearest swarm down a dimensional pit. Olofire removes a jar of alchemist's fire and douses one of the swarms, leaving the worms greatly diminished (it is likely next round this swarm will die to the burn damage - just don't roll minimum damage).

    Shukak is done speaking, and lashes out at Lydia and Frederick in a frenzy! He strikes twice at Frederick in rapid succession with his trident (missing once, 24 damage), then he bites and claws at Lydia, missing with the claw (22 damage). His massive wings beat at the air as he rises up, his intent to get some altitude on his foes before his next strike (5-ft step directly up). Both Frederick and Lydia can see that his previous wounds have not healed, but instead now drip large globs of blood with green worms inside, suggesting some foul corruption within his transformed body.

    The worm swarms make no deliberate noise, though from close enough the sound of their segmented bodies sliding over each other is disturbing to say the least. None move quickly (5-ft each round), but they all move towards the PCs as best they are able. It looks like Jan's pits will hold them for now, but one is adjacent to Frederick, threatening him with its attacks next round and the one nearest the summoned centipede swarms over it (dealing 6 damage).

    The map is updated. Begin round 7!

    Amearin's spell completes, and a writhing mass of spiders appears on the ground right next to the singed batch of worms assailing Olofire! As the arachnids begin pouring over the worms he calls out a warning: "Olofire buddy - back away! I can't control them!" Finally, he steps away from the edge of the pit and starts another summon, looking fearfully over his shoulder at Shukak...

    Across the room, the giant centipede is in dire straights as dozens of worms are biting and writhing through its flesh. It continues to thrash about, trying to destroy as many of them as it can before it is unmade.

    The centipede will do its best to keep damaging the swarm in S/24 with its smite evil ability:

    The spider swarm appears in F/19 and goes after the nearest living thing, which is the worm swarm in F/20. It moves into F/20 (can two swarms share a square?) and does (1d6)[2] swarm damage to the worms. (I'm guessing they're immune to the poison and distraction effects.)

    After this the spider swarm isn't under Amearin's control, and will mindlessly move to attack the nearest target. (Hopefully that's the other swarm in I/18 and not one of us!)
