No, its probably new to you. So don't worry about it (well, more than a note from a monstrous killer should cause worry)

You're right. I've swapped it for Steady Balance. I planned on taking that since I can take a rogue feat that everybody who is frightened is flat-footed for me (meaning sneak attack). but I think I'll first take the rogue feat that gives me flanking bonus whenever I'm together with someone, regardless of position. That's also a very good one for a rogue.

Don't know if it might be useful and change your plans, but Lysander can inflict the Flat-footed condition with his attacks (he just needs to hit and the enemy is flat-footed).

I would say no. The persistent damage would not be doubled. Its a rider not the initial effect.
